Gli apporti venuti alla psicologia dalla teoria dei giochi, la sociologia e l’economia sperimentale hanno creato negli ultimi quarant’anni un connubio felice tra queste scienze e la psicologia. Ne è emersa una nuova antropologia: gli esseri umani non sono né naturalmente buoni né naturalmente cattivi, e sono naturalmente portati a forme di socialità, di cooperazione, e perfino di altruismo, oltre che ovviamente anche alla competizione e talora alla distruttività. Vanno però distinte due forme di altruismo. La più elementare è la forma di altruismo finalizzato alla conservazione del gruppo dei consanguinei. Una forma di altruismo più evoluto è quello reciproco, e riguarda la cooperazione “a rischio” tra individui non imparentati, e talora perfino fra estranei. È questa una forma di cooperazione che prende forma col diffondersi di strategie di fiducia “allargate” (cioè “fra estranei”) in grado di prevalere sui solidarismi tribali tradizionali e sui sistemi localistici e clientelari. Nel mondo della modernità e della globalizzazione, la possibilità di decollo socioeconomico di una nazione dipende in misura cospicua dal diffondersi di questa forma di cooperazione “aperta” o “allargata”. Il familismo, il clientelismo, la cultura della raccomandazione, i padrinaggi fanno parte di una cultura premoderna che non produce né senso critico, né emancipazione per i giovani e le donne, e neppure benessere economico.

When the wind of the 1960s blew through the world of psychiatry In 1961, when Franco Basaglia arrived outside the grim walls of the Gorizia asylum, on the Italian border with Yugoslavia, it was a place of horror, a Bedlam for the mentally sick and excluded, redolent of Basaglia’s own wartime experience inside a fascist gaol. Patients were frequently restrained for long periods, and therapy was largely a matter of electric and insulin shocks. The corridors stank, and for many of the interned the doors were locked for life. This was a concentration camp, not a hospital. Basaglia, the new Director, was expected to practise all the skills of oppression in which he had been schooled, but he would have none of this. The place had to be closed down by opening it up from the inside, bringing freedom and democracy to the patients, the nurses and the psychiatrists working in that “total institution.” Inspired by the writings of authors such as Primo Levi, R.D. Laing, Erving Goffman, Michel Foucault and Frantz Fanon, and the practices of experimental therapeutic communities in the UK, Basaglia’s seminal work as a psychiatrist and campaigner in Gorizia, Parma and Trieste fed into and substantially contributed to the national and international movement of 1968. In 1978 a law was passed (the “Basaglia law”) which sanctioned the closure of the entire Italian asylum system. The first comprehensive study of this revolutionary approach to mental health care, The Man Who Closed the Asylums is a gripping account of one of the most influential movements in twentieth-century psychiatry, which helped to transform the way we see mental illness. The 1978 Italian Psychiatric Reform was welcomed as a significant advancement in the care of the mentally ill, as it involved, for the first time ever, the complete shutdown of psychiatric hospitals in a major Western country. Today, Italian psychiatry is totally different from that of the rest of the world, due to its complete commitment to community care. The transition towards the community model was appraised by many relevant international organisations, such as the World Health Organisation, as a fundamental step towards a better quality of life, well-being and social functioning of persons with mental diseases. This passage wasn’t easy, however, and the closure of Italian psychiatric hospitals was accompanied by notable setbacks in the treatment of the most severely affected persons, who often faced the inadequacy of a ‘crisis management’ system of care rejecting interventions in the long-term. In past decades, pro-reform authors also tended to refuse criticism of such obstacles, due to their extreme commitment towards the principles that inspired their practice. This book provides a much-needed appraisal of the 1970s Italian Psychiatric Reform. With an independent viewpoint, it highlights the often-overlooked shortcomings of the reform, while also presenting a multi-faceted view in contrast with the ‘single-vision’ attitude often adopted in existing studies on this topic.
