"The current epidemic of Type-2 diabetes among Aboriginal Peoples in Canada has been associated with the poor socioeconomic status of Aboriginal Peoples. Most strategies in diabetes prevention treat health behaviour as an individual choice and ignore how the social determinants of health shape people’s health behaviour. The purpose of this project is to explore how NPs might more effectively approach diabetes prevention in Aboriginal communities. This integrative literature review includes a critical analysis of Aboriginal community-based diabetes prevention strategies. A synthesis of their shared characteristics shows that effective community-based diabetes prevention strategies are congruent with primary health care and social justice perspectives on health, which focus on empowering communities and correcting institutionalized discrimination. Community-based strategies empower communities to take control of health promotion efforts and improve community resources and infrastructures that support healthier lifestyles. With an understanding of community-based programs, NPs can begin identifying how primary health care and social justice perspectives can strengthen existing diabetes prevention efforts in Aboriginal communities."--P. i.

The purpose of this study was to explore urban Aboriginal people's perspectives on diabetes and their experiences using health services. Using a qualitative approach, in-depth semi-structured interviews were conducted with eight urban Aboriginal people with diabetes. The data generated were analyzed using content analysis. The findings indicate that urban Aboriginal people have difficulty accessing and navigating health care services as result of social, cultural, and economic circumstances. Further, lack of resources and information appropriate to participants' needs created difficulties in their making lifestyle changes for diabetes management. Emphasized was the need for greater diabetes awareness and availability of suitable resources for the Aboriginal community. In the discussion, the new paradigm of chronic disease management, with emphasis on self-management, is explored as an approach to change the health care system and make improvements to diabetes care services that positively contribute to Aboriginal people's health. Based on the results, implications for programs and services, health care providers, and further research are presented.

"Inequalities in health outcomes in Canada are evident when we look at chronic health conditions, diabetes in particular. Aboriginal communities and lower-income people in Canada are at particularly high risk for developing a range of chronic health conditions including diabetes. Depending on the age group, First Nations adults are two to eight times more likely to have diabetes than Canadians generally, and they also have substantially higher rates of serious complications from diabetes, including kidney failure, foot amputations, heart disease and infectious disease. In this report, the Health Council profiles an initiative in northern Manitoba - the Island Lake Regional Renal Health Program - developed in response to the high rates of kidney failure in a remote First Nations region. And we take a look at the Kahnawake Schools Diabetes Prevention Program which, after 12 years of sustained effort, has put the brakes to the persistent increase in new cases of diabetes in that Mohawk First Nation community near Montreal. The connection between health and wealth is also important in understanding how to influence health outcomes. The poorest Canadians are almost three times as likely to have multiple chronic health conditions, including diabetes, as the highest-income Canadians, and the ratio steps down steadily as family income goes up."--Page 12
