Despite improvements over the last decade or more, India still accounts for a large proportion of the global prevalence of maternal and child undernutrition. We use a cluster-randomized controlled design and two waves of panel data on more than 2000 households from Bihar to analyse the impact on diet quality and anthropometry of a health and nutrition intervention delivered through an at-scale women’s self-help group (SHGs) platform. We find that the intervention had small but significant impacts on women and children’s dietary diversity, with the main impacts coming from an increase in the consumption of fruits and vegetables and dairy, however, it had no impact on women’s body mass index. We identify several potential pathways to impact. To the extent that SHGs can effect broad-based social change, their current reach to millions of women makes them a powerful platform for accelerating improvements in maternal and child health and nutrition outcomes.

"BACKGROUND:Undernutrition is one of the leading causes of death among children worldwide, estimated to have contributed to nearly half of under-5 deaths in 2019. Sub-Saharan Africa is the region with the highest prevalence of moderate-to-severe food insecurity at 55.6% and the highest prevalence of under-5 chronic malnutrition, or stunting, at 42.9%. Limited research has been conducted on the relationship between women's agency and women's and children's nutrition and food security status in sub-Saharan Africa. OBJECTIVES:The present thesis consists of three objectives: (I) develop context-specific models of women's agency; (II) estimate the association between women's agency and (a) women's and children's nutrition and (b) women's food insecurity status; and (III) estimate the association between women's and men's concordance on notions of women's agency and (a) women's and men's dietary diversity and (b) women's food insecurity status. METHODS:The data used for this thesis comes from a cross-sectional survey in Ethiopia, Malawi, Mozambique, and Zambia. In total, 1,261 households in Ethiopia, 708 households in Malawi, 735 households in Zambia, and 1,262 households in Mozambique were surveyed. I used confirmatory factor analysis to build country-specific measurement models for women's agency. I estimated agency scores that were used to model the association between women's agency and women's and children's nutrition and food security. For the third objective, I estimated the association between couples' agreement on domains of women's agency and women's and men's dietary diversity and women's food insecurity experience. RESULTS:The best-fitting models estimated for women's agency in Objective I were different for each country; and domains of agency were not always correlated with conventional measures of women's empowerment. The analyses conducted for Objective II yielded mixed results for the association between women's agency and women's and children's nutrition and food security outcomes. For women's nutrition, the strongest associations were found between women's decision-making and women's nutrition status, with the relationship being positive in some instances and negative in others. Decision-making was associated with an increased risk of children's malnutrition in some countries and a decreased risk in others. While agency was consistently associated with increased dietary diversity in women and children, results for the association between women's agency and women's food insecurity experience were mixed. In my third study, domestic partner concordance on gender-based attitudes improved dietary diversity for women and men in three of the four countries, but was not associated with women's food insecurity experience. Lastly, partner concordance on women's decision-making was differentially associated with women's and men's dietary diversity and women's food insecurity experience both within and between countries. CONCLUSIONS:The findings of my study support a shift away from standardized measures of women's agency towards more nuanced, context-specific and, most importantly, culturally valid alternatives. Results for the association between domains of women's agency and measures of nutrition, dietary diversity, and food insecurity were mixed. The variation of findings--between countries and between different domains of agency in a single country--supports the notion that a given construct of agency can represent distinct phenomena in different settings. Further, my results support the treatment of anthropometry, dietary diversity, and food insecurity as separate, yet interrelated facets of nutrition. Future research would benefit from a more in-depth understanding of how women internalize theoretical constructs of agency and, subsequently, how assertions of agency impact women's and children's nutrition and food security status"--

Investments in social assistance programs (SAPs) in low- and middle-income countries (LMICs) are increasing. As investments increase, the objectives of these programs are expanding from focusing on reducing poverty to addressing other social issues such as improving diets and nutrition. At the same time, there is increasing interest in addressing all forms of malnutrition within the framework of food systems. Given the intersections between SAPs and food systems, we reviewed the effectiveness of SAPs (agriculture asset transfers, cash transfers, in-kind transfers, vouchers, public works and school meals programs) for reducing all forms of malnutrition across the lifecycle within a food systems framework. As several programs included multiple treatment arms, each representing a unique program design, we used study arm as the unit of analysis and assessed the proportion of study arms with positive or negative program impacts on diet and nutrition outcomes among men, women and children. The majority of the studies included in this review were from evaluations of agriculture asset, cash and in-kind transfer programs. There was clear evidence of positive impacts on women’s and children’s diet-related outcomes. Very few studies assessed program impact on women’s nutritional status outcomes. However, there was some evidence of impacts on increasing body mass index and hemoglobin concentration (Hb) with in-kind transfer programs. Among children, several study arms across the agriculture asset, cash and in-kind transfer programs found positive impacts on increasing height-for-age Z-score (33%-45% of study arms) and weight-for-height Z-score (33%-50% of study arms) and decreasing the prevalence of wasting (43%-60% of study arms). Cash and in-kind transfer programs also found positive effects on reducing stunting prevalence in 33% and 45% of study arms, respectively. Lastly, a few study arms assessed program impact on increasing Hb with some evidence of positive impacts in in-kind and school feeding programs. There was a paucity of relevant evidence of the effectiveness of voucher and public works programs on diet and nutrition outcomes, for men’s outcomes and on micronutrient status. Several challenges remain in understanding the potential for SAPs to improve diet and nutrition outcomes within food systems including the heterogeneity of program and evaluation designs, populations targeted by the programs and included in evaluations and indicators used to assess impact. Addressing these challenges in future evaluations is important for informing program and policy actions to improve the effectiveness of SAPs within food systems for improving diet and nutrition outcomes across the lifecycle.

1.1 Background Bangladesh has made commendable progress in domestic food production through public investments in agricultural research and extension, public and private investments in irrigation, and liberalization of agricultural input markets. In the early 1970s, Bangladesh was a food-deficit country with a population of about 75 million people. Today, the population has more than doubled, and the country is nearly self-sufficient in rice production, which has tripled over the past three decades. However, Bangladesh’s performance in improving child and maternal nutrition has been less satisfactory. Despite its success in reducing child stunting, the rate of stunting in Bangladesh (36 percent in 2014) remains high (NIPORT 2015). Bangladesh also continues to struggle with deficiencies in micronutrients such as iron, zinc, iodine, and vitamin A. Such deficiencies reflect poor diets that are rice-dominated, monotonous, and lacking diversity (Ahmed et al. 2013). Anemia (in part due to iron deficiency) is estimated to affect 26 percent of nonpregnant, non-lactating women, whereas 42 percent suffer from iodine deficiency. About 28 percent women of reproductive age are underweight (NIPORT 2015). In preschool children, the rates of anemia, iodine, and vitamin A deficiencies are 33 percent, 40 percent, and 20 percent, respectively (ICDDR,B 2013). Therefore, government policies and strategies underscore the importance of strengthening the linkage between agriculture and nutrition. Agriculture provides a source of food and nutrients, contributes to income, and affects food prices. Exploring agriculture and nutrition linkages in Bangladesh using data from a multi-round district level panel, a study finds that rice yields are associated with earlier introduction of complementary foods to young children, as well as increases in their weight-for-height (Heady and Hoddinott 2016). Agriculture can also have effects on women’s health, nutrition, empowerment and time allocation, which can have important consequences for their ability to care for family members. Given these links, agriculture has the potential to be a strong driver of nutrition. However, that potential is not being fully realized in Bangladesh because, traditionally, nutrition and agricultural policies have been uncoordinated. Low status of women and gender gaps in health and education contribute to chronic child undernutrition (Smith et al. 2003) and food insecurity (von Grebmer et al. 2009), even when other determinants of food security, such as per capita incomes, improve. According to an IFPRI study, women are key actors within the food system, but are historically disempowered in Bangladesh in terms of leadership in the community, control of resources, and control of income (Sraboni, Quisumbing, and Ahmed 2014a). The lack of women’s empowerment weakens the links between agriculture and nutrition. Despite increases in 2 women’s participation in agriculture in Bangladesh in recent years (Asaduzzaman 2010), women face persistent obstacles, particularly due to social and economic constraints, which limit their further inclusion in agriculture. Women have limited control over agricultural assets, as well as limited mobility to go to markets to sell agricultural produce, often relying on husbands and sons to take produce to market. 1.2 Motivation for the Study IFPRI research in Bangladesh, using data from a nationally representative household survey conducted by IFPRI, reveals that women’s empowerment plays a key role in improving household food security and dietary diversity of children, women, and other household members (Sraboni et al. 2014b; Malapit et al. 2015). The study also shows that agricultural production diversity is associated with dietary diversity (Sraboni et al. 2014b). Further, IFPRI research in Bangladesh shows that nutrition behavior change communication (BCC) training imparted to women and men in rural households leads to significant improvements in child nutrition and complementary feeding practices (Ahmed et al. 2016; Menon et al. 2016). Motivated by research-based evidence, IFPRI researchers developed a concept note to strengthen the agriculture-nutrition-gender nexus in Bangladesh and presented it to the Ministry of Agriculture (MOA), Government of the People’s Republic of Bangladesh in June 2014. Based on the concept note, an inter-ministerial committee of the Government of Bangladesh approved a pilot research project entitled, “Orienting Agriculture Toward Improved Nutrition and Women’s Empowerment”, also known as “Agriculture, Nutrition, and Gender Linkages”(ANGeL), for implementation by the MOA, with technical assistance from IFPRI and Helen Keller International (HKI), and an evaluation led by IFPRI. The Minister of Agriculture officially launched the pilot project in October 2015. The project is jointly funded by the Government of Bangladesh and USAID. 1.3 The Baseline Report As part of the evaluation of the ANGeL Project, IFPRI carried out a baseline survey of project participants and a comparison group of households just before the start of project interventions. This report presents the results of the ANGeL baseline survey. It is organized in nine sections. Section 2 describes the salient features of the ANGeL Project. Section 3 presents the progress of the ANGeL Project to date. Section 4 describes the baseline survey. Section 5 gives a profile of the survey households. Section 6 provides the land tenure status of sample households and findings on agricultural production and practices. Section 7 presents patterns of food consumption and nutrition. Section 8 provides findings on women’s empowerment. Section 9 summarizes the main findings and provides conclusions.

The 2020 report on the State of Food Security and Nutrition in the Asia and Pacific region, provides an update on progress towards the 2030 targets (SDGs and WHA) at the regional and country level. Selected indicators look at undernourishment, food insecurity, childhood stunting, wasting and overweight, adult overweight, child minimum acceptable diet, exclusive and continued breastfeeding, and anaemia in women and children. While the region continues to work towards ending all forms of malnutrition and achieving Zero Hunger, progress on food security and nutrition has slowed, and the Asia and Pacific region is not on track to achieving 2030 targets. About 350.6 million people in the Asia and Pacific region are estimated to have been undernourished in 2019, about 51 percent of the global total. An estimated 74.5 million children under five years of age were stunted and a total of 31.5 million were wasted in the Asia and Pacific region. The majority of these children in the region live in Southern Asia with 55.9 million stunted and 25.2 million wasted children. Estimates predict a 14.3 percent increase in the prevalence of moderate or severe wasting among children under 5 years of age, equal to an additional 6.7 million children, due to the COVID-19 pandemic. With basic food prices and disposable incomes influencing household decisions on food and dietary intake, they are critical to improve food security and nutrition in the region. However, in the Asia and Pacific region, 1.9 billion people are unable to afford a healthy diet, driven by high prices of fruits, vegetables and dairy products, making it impossible for the poor to achieve healthy diets.In Part 2, the 2020 report promotes a systems approach to healthy maternal and child diets, involving and coordinating institutions and actors in the Food, Water and Sanitation, Health, Social Protection and Education systems, to collectively create the enabling environment for healthy diets. Integration of healthy diets and nutrition-focused Social Behavior Change Communication (SBCC) mainstreamed throughout these systems will lead to greater uptake and sustainability of healthy behaviours and caregiver’s knowledge.
