This monograph examines the feasibility of a new model for maintaining the clinical skills of the military medical force. Under the model, active-duty personnel would be assigned to civilian settings during peacetime. The study on which this monograph is based explored the feasibility of this model from a civilian perspective, focusing on civilian receptiveness to the proposed arrangement and identifying potential barriers and concerns. The study found that civilian medical organizations are generally receptive to the idea of such a model and that DoD could consider conducting a pilot study to assess the effectiveness of the model in improving military medical readiness.

The DoD implemented the post-deployment health reassessment (PDHRA), which is required to be administered to servicemembers 90 to 180 days after their return from deployment. DoD established the PDHRA program to identify and address servicemembers¿ health concerns that emerge over time following deployments. Posttraumatic Stress Disorder (PTSD) is a chronic, burdensome disorder that deserves more attention as military service members continue to deploy to tcurrentent conflict in the Middle East. Studies are needed to identify what factors contribute to the onset of PTSD and also to identify those individuals most vulnerable to onset. This study provides information to help healthcare professionals identify the onset of PTSD, even before service members report symptoms of PTSD. Data from the Army Medical Surveillance Agency's (AMSA) Defense Medical Surveillance System (DMSS) database, which houses health assessment deployment data for all military personnel, were used in the current study. The data were derived from three forms: (1) Department of Defense (DD) Form 2795 (completed within 30 days prior to deployment), (2) DD Form 2796 (completed 30 days prior to leaving a deployed location or within 30 days after returning from a deployed location), and (3) DD Form 2900 (completed 90 to 365 days after returning from a deployed location). Participants included 5,045 male Army service members who completed DD2900 between August 2005 and April 2006 and reported at least one symptom of PTSD. Service members were categorized into three groups: (1) reported symptoms of PTSD only on DD2796, (2) reported symptoms of PTSD only on DD2900, and (3) reported symptoms of PTSD on both DD2796 and DD2900. While all service members in this study reported symptoms of PTSD on either DD2796 or DD2900, only 11% of service members indicated they were interested in receiving help for stress on DD2796 and 16% of service members on DD2900. Additionally, this study found a significant decrease in general health reported over time for all service members. Thus, the responsibility of identifying PTSD may fall on healthcare professionals as service members may be more willing to seek treatment for physical symptoms, and less willing to seek treatment for mental health. Interestingly, service members who reported an intent to seek mental health services on DD2796, did not report an increase in symptoms of PTSD over time. Healthcare professionals should also consider the differences between the three PTSD groups identified in this study. All three PTSD groups differed from one another on reported physical health symptoms on DD2796. Furthermore, a combination of responses and physical symptoms were identified that distinguish between the three PTSD groups, and a sample PTSD screening questionnaire was created accordingly. It is recommended that healthcare professionals use the screening questionnaire to ask questions initially following deployment that identify service members at risk for PTSD.

The U.S. military has been continuously engaged in foreign conflicts for over two decades. The strains that these deployments, the associated increases in operational tempo, and the general challenges of military life affect not only service members but also the people who depend on them and who support them as they support the nation â€" their families. Family members provide support to service members while they serve or when they have difficulties; family problems can interfere with the ability of service members to deploy or remain in theater; and family members are central influences on whether members continue to serve. In addition, rising family diversity and complexity will likely increase the difficulty of creating military policies, programs and practices that adequately support families in the performance of military duties. Strengthening the Military Family Readiness System for a Changing American Society examines the challenges and opportunities facing military families and what is known about effective strategies for supporting and protecting military children and families, as well as lessons to be learned from these experiences. This report offers recommendations regarding what is needed to strengthen the support system for military families.
