Originally published in 2004. Providing Integrated Health and Social Care for Older Persons - Issues, Problems and Solutions (PROCARE)" is a project in the EU Fifth Framework Programme (Quality of Life and Management of Living Resources, Area "The Ageing Population and Disabilities") that aims to help in defining the new concept of an integrated health and social care for older persons in need of care by comparing and evaluating different modes of care delivery. The project will identify structural, organisational, economic and social-cultural factors and actors that constitute an integrated and sustainable care system with enhanced outcomes for all actors involved. This book gathers the achievements of the first project phase (2002) that consisted in a literature overview focusing on the question which of the variety of innovations in modes of organisation, finance and professional collaboration observed in Europe over the last decade have been the most successful and long-lasting ones. Thus, national reports from nine EU Member States (Austria, Denmark, Finland, France, Germany, Greece, Italy, the Netherlands and the UK) will be presented by scholars from leading research and consulting agencies in these countries. The national reports follow a mutually agreed structure. The publication is introduced by a general overview and a more theoretic article defining the issues at stake. The book gives a unique general overview on European approaches towards integrated social and health care services and policies that are to be developed to face the growing need of care in ageing societies; furthermore, it provides indicators for successful approaches and models of good practice to overcome the "social-health-divide" and a better understanding of the meaning of integrated services and coordination of social and health systems in the different countries. Demographic, epidemiological, social and cultural trends in European countries are changing the traditional patterns of care. The next decades will see rising numbers of care-dependent older people and noncommunicable diseases as the leading cause of chronic illness and disability. Further, urbanization and the break-up of the traditional large family group will lead to gaps in the care of older or disabled family members. These changes in needs and social structure require a different approach to policy and services in the health and social sectors; a disease-oriented approach alone is no longer appropriate. Patient mobility across Europe is markedly increasing and new generations will actively ask to be treated by the health-care system that best meets their needs. At a political level, the EU issued the EU Directive no. 24/2011/CE of 9th March 2011 concerning the application of patients’ rights in cross-border health care and has contributed to improving the level of freedom of choice for the European citizen, but it does not seem to have increased actual patient mobility across Europe. Freedom to choose is necessary to grant the people of Europe the same access to public-sector health-care services. The latter is a key instrument for an efficiently functioning “single market” ensuring real mobility within the EU. The aim of this book is to study the current European health care market and discuss the hypothesis of a European right of citizenship with reference to health-care services. It examines patients' mobility from several perspectives: determinants of patient mobility, governance of cross-border mobility at EU level as concerns patients and health-care professionals, policy implications, and case studies. It is intended for health researchers, decision-makers and professionals concerned with health-care provision and patient mobility. The goal is to provide, through scientific and methodological rigor, new informative tools useful for the implementation of new policies in the health-care sector in order to implement effective health-care integration in the European Union.
