The governance of long-term care is fragmented between the Ministry of Health, the Ministry of Social Security and Labour and the municipalities.

Originally published in 2004. Providing Integrated Health and Social Care for Older Persons - Issues, Problems and Solutions (PROCARE)" is a project in the EU Fifth Framework Programme (Quality of Life and Management of Living Resources, Area "The Ageing Population and Disabilities") that aims to help in defining the new concept of an integrated health and social care for older persons in need of care by comparing and evaluating different modes of care delivery. The project will identify structural, organisational, economic and social-cultural factors and actors that constitute an integrated and sustainable care system with enhanced outcomes for all actors involved. This book gathers the achievements of the first project phase (2002) that consisted in a literature overview focusing on the question which of the variety of innovations in modes of organisation, finance and professional collaboration observed in Europe over the last decade have been the most successful and long-lasting ones. Thus, national reports from nine EU Member States (Austria, Denmark, Finland, France, Germany, Greece, Italy, the Netherlands and the UK) will be presented by scholars from leading research and consulting agencies in these countries. The national reports follow a mutually agreed structure. The publication is introduced by a general overview and a more theoretic article defining the issues at stake. The book gives a unique general overview on European approaches towards integrated social and health care services and policies that are to be developed to face the growing need of care in ageing societies; furthermore, it provides indicators for successful approaches and models of good practice to overcome the "social-health-divide" and a better understanding of the meaning of integrated services and coordination of social and health systems in the different countries. Finally, facts and figures about coordination at the interface between health and social care for older persons as well as problems and solutions (

We live in a rapidly aging world, in which people who are age 60 and older outnumber children under the age of five. This book reveals large and growing gaps in care for older adults in countries at all income levels and shows how to leverage reforms for improving health outcomes for older adults and create healthier, more prosperous communities. Aimed at policy makers and other health and development stakeholders who want to promote healthier aging, Silver Opportunity compiles the latest evidence on care needs and gaps for aging populations. It argues that primary health care should be the cornerstone of integrated service delivery for older people, but primary health care systems must first build their capacity to respond to older people’s health needs. It presents an original framework for policy action to advance primary health care†“centered, integrated senior care; documents the experiences of pioneering countries in delivering community-based care to older people; and provides recommendations for decision-makers. The framework presents four policy levers with which to improve health care for seniors—financing, innovation, regulation, and evaluation and measurement—or FIRE. Finally, the book posits that by acting now, countries can leverage population aging to accelerate progress toward health equity and universal health coverage.

This book is about older people, and the contribution occupational therapy can make towards the maintenance of their health and the appropriate provision of rehabilitation and care. The aim is to help occupational therapists and those involved in the commissioning of occupational therapy services for older people to target their efforts both sensitively and effectively, and to anticipate the demands which will shape service provision in the future. The entire work is underpinned by current evidence regarding best practice and opinions voiced by older people during several research projects undertaken by the author. The first chapters examine the experience of growing older, from both societal and individual perspectives, the factors contributing towards vulnerability in older age and the provision of services to meet needs. The second part of the book concentrates upon the occupational therapy assessment and treatment processes. The final chapter considers the challenges for occupational therapy.

Highly Commended in the Public Health category of the 2003 BMA Medical Book Competition. 'The book is also helpfully laid out. `Signposts' in right hand margins link issues discussed in different parts of the book. Each chapter has a short summary of contents at the start, an overview at the end and `headline points'- short bullet points designed to reinforce key messages in the reader's mind. The end of each chapter also includes references and further suggested reading... This book will repay study by anyone involved in planning or delivering services for older people- particularly those working to try to implement `joined up' services. From this perspective, the book is genuinely inspirational. It shows how much can be achieved by working in this way and -more importantly- how it was done.' - Journal of Interprofessional Care 'This is an important book... It needs to be read and understood by everyone involved in caring, but especially by those with the power to influence change- which is increasingly purchasers and senior managers. It also provides a valuable guide to a large literature and has useful examples of questionnaires, data collection forms and clinical vignettes.' - International Journal of Geriatric Psychiatry 'This book is unique and brilliant at the same time. It shows the limits of what any given healthcare professional can accomplish and the amazing, seemingly impossible feats that an effective team caring for older persons can accomplish. The commentary on leadership in healthcare and the ways in which healthcare teams evolve and work is excellent. It is not in the least dry, but rather invigorating as one sees the concepts, values, and visions manifest in clever solutions to problems... It weaves in the history of geriatrics seamlessly. The enthusiastic anticipation in the excellent foreword is amply justified. The concepts here are universal... Every medical library should possess this remarkable book.' - Doody's Reviews The authors of this book share their practical experience of implementing a multi-agency approach to the support of older people. They show how systems thinking can help with the complexity of developing a model of care that co-ordinates medical, social and community services. They offer insight into the effective use of on-going assessment, evaluation, costing and information technology, with examples. They demonstrate how they achieved a `one-stop' rapid response to emergencies and a multi-level approach to risk management. They also show how enabling the older person to become an equal partner in decision-making, both as an individual user and as a citizen, reduces dependency, and can prevent current problems such as overlap and duplication of service and inappropriate admission to hospital and residential care. They offer models of consultation and guidance on running participative groups. This resource meets the growing need for material on multi-agency practice and is an invaluable tool for all those working across organisational and professional boundaries to deliver an integrated care system for older people.
