L'acquisition d'un fonds de commerce de pharmacie d'officine est un investissement considérable, tant financier que personnel. Cette thèse reprend chaque étape du projet d'installation, depuis l'étude de marché jusqu'à la signature de l'acte définitif de vente, en passant par la recherche de financement et le choix du montage juridique le plus adapté. Nous nous sommes intéressés au fonctionnement des pharmacies de stations de ski afin d'identifier les particularités de gestion à prendre en compte lors du projet d'acquisition. Une enquête qualitative a été réalisée auprès de six pharmaciens titulaires installés dans le domaine skiable des 3 Vallées en Savoie. L'analyse thématique des entretiens montre que toute la gestion de ces officines est affectée par la saisonnalité. Les importantes variations de population impactent le chiffre d'affaires réalisé, la masse salariale et le prévisionnel d'activité à construire. La majorité du chiffre d'affaires est réalisé durant la saison d'hiver et doit couvrir des charges d'exploitations annuelles et potentiellement plus élevées qu'ailleurs, ce qui explique notamment un taux de marge supérieur à la moyenne. Ceci conduit à une gestion des achats et de la trésorerie propres à cette activité saisonnière. Avant de s'installer en station de ski, le pharmacien devra être avisé quant à ces modalités d'exercice et tenir compte de toutes ces notions lors de l'élaboration du business plan. Toutefois, la fluctuation du nombre de médecins en station de ski soulève un problème d'accès aux soins pour la population locale. Quelles missions et services ces pharmacies seront-elles amenées à développer en tant qu'acteur de santé de proximité ?

Chaque année, des milliers de vacanciers venus de tous horizons affluent en direction des stations de sports d'hiver à la recherche de dépaysement, de repos, mais principalement d'aventures et bien évidement de sensations fortes. Bien souvent, pour un grand nombre d'entre eux, la pratique de sports de glisse est un rendez-vous annuel. Naturellement, par manque d'expériences, d'entraînements, de connaissances ou bien encore de vigilance ce milieu montagnard peut vite devenir très hostile et accidentogène. Inévitablement, en association à des conditions climatiques extrêmes et très changeantes, une multitude de maux tant bénins que mortels surviennent chaque saison. Ainsi, on imagine aisément que l'exercice d'une officine de montagne doit être bien différent de l'exercice d'une officine située en vallée. Pour objectiver ce ressenti, nous avons mené une étude de la spécificité de patientèle et d'activité d'une pharmacie de haute montagne à Tignes, en comparaison avec une pharmacie de vallée à Rambervillers. Nous avons également identifié les prises en charge des pathologies et demandes spécifiques des patients liés à la vie en milieu montagnard, en détaillant les origines, les traitements et les conseils que le pharmacien doit prodiguer. Ce travail lui permettra d'être une référence en la matière tout en proposant des fiches synthétiques (avec le vocabulaire anglais afférent) sur lesquelles il pourra s'appuyer pour les demandes les plus souvent rencontrées au comptoir.
