This volume arose from a European Scientific Seminar on `Strategies and Policies to Combat Drugs', which was organized by the Commission of the European Communities at the European University Institute in Florence in December 1993. The significance of the seminar lay in the manner in which it sought to address the full range of issues associated with the drug problem at international, national and local level. Equally important was its success in attracting participants from a number of different disciplines -- scientific experts, national and EU administrators working in the drugs field, and members of the European Parliament. That provided for a lively and wide-ranging exchange of views and ideas which is reflected in the contributions and conclusions contained in this publication, which comprises varied contributions and approaches to drug policies at international, European, national and local level. It also focuses on the possibilities of a comprehensive Drug Strategy within the framework of the European Union.

Background to multi-city study : The multi-city study began in 1983. The objectives of the study are both methodological - to improve the quality, usefulness and comparability of indicators of drug misuse, and substantive - to monitor and interpret trends in drug misuse across a network of major European cities. The first report (1987) included a critical methodological review of different indicators, and provided data on seven cities up to 1985. Aims of this report : Since 1985, social and political changes have taken place in Europe, drug misuse patterns and drug policies have evolved, AIDS has become a major issue, and the Expert Epidemiology Group has expanded. The present report focusses on comparative trends in thirteen cities over the 1980s, and especially from 1985 to 1991. Geneva : Growth of heroin addiction started in 1970s and accelerated over 1980s. Current trends not clear, through death rate rising and heroin price falling. Cocaine use increased, as has use of benzodiazepines combined with other drugs and alcohol. High level of HIV infection in longer term users, through stable in methadone maintained clients and declining in more recent cases. Major change in policy towards harm reduction from mid 1980s
