Los primeros casos comunicados en Wuhan (China) en Diciembre de 2019, de neumonía grave por SARS-CoV-2, y su rápida propagación alertaron a las comunidades científicas internacionales de la gravedad de la enfermedad hasta su declaración por la OMS como pandemia el día 11 de marzo de 2020.A raíz de la pandemia COVID-19 se replantearon y alteraron muchos paradigmas de la cirugía y, más concretamente, la asistencia al paciente quirúrgico. En nuestro país, la pandemia tuvo su punto más virulento en los meses de Marzo y Abril de 2020, iniciándose a principios de Abril la estabilización de la curva ascendente de casos. Desafortunadamente, actualmente estamos asistiendo a una nueva escalada de rebrotes y aumento generalizado de nuevos casos que no auguran una remisión estable y progresiva de la pandemia.Durante estos meses, toda la actividad médica y quirúrgica del hospital sufrió una profunda remodelación de sus puestos tradicionales y modelos de trabajo, afectando a la distribución del personal sanitario asistencial, auxiliar y administrativo y con cambios muy señalados en la reubicación de los puestos de trabajo asignados a los cirujanos; esto conllevo, entre otras modificaciones, a una disminución en la asignación de quirófanos para la cirugía programada y para la cirugía de urgencia, que precisó de una individualización, aislamientos y reasignación de quirófanos para pacientes COVID-19 y no COVID-19.La rápida velocidad de propagación de la enfermedad también obligo a los responsables del hospital a una inmediata reorientación de los recursos sanitarios disponibles, fundamentalmente las urgencias, laboratorios y UCI. El periodo de adaptación fue agobiante para el personal sanitario de los hospitales con alta incidencia de casos urgentes COVID-19, que tuvieron que transformarse y modificar protocolos y guías según los cambios suscitados en cada momento. De manera particular, cada Servicio médico o quirúrgico tuvo que adecuarse a la situación imperante en cada instante y los servicios quirúrgicos tuvieron que distribuir a sus cirujanos para desarrollar las funciones que eran más demandadas.Los capítulos hacen referencia y se circunscriben a la experiencia vivida en el Servicio de Cirugía General, Aparato Digestivo y Trasplante de Órganos Abdominales del Hospital Universitario 12 de Octubre, durante el periodo limitado e inicial de máxima expansión de casos de la pandemia por COVID-19 en nuestro país, durante 2020. Asimismo, se ha seguido el guión establecido en las primeras Reuniones Médico-Quirúrgic@s “on-line” desarrolladas en Junio de 2020.“La Cirugía General y Digestivo en el inicio de la Pandemia por COVID-19 (SARS-COV-2): Un escenario real e inesperado”, pretende acercar al lector a diferentes ambientes reales vividos por sus autores, con un carácter eminentemente divulgativo y fundamentado en la práctica clínica asistencial y de la cirugía urgente y oncológica.
