On meurt et on mourra de plus en plus à l'hôpital. 70 à 80 % des individus viennent y mourir, de maladie ou de vieillesse. C'est un fait de société, qui amène aussitôt la question : l'hôpital, conçu pour guérir, est-il aménagé pour accueillir les mourants ? L'hôpital d'autrefois, ou "hospice" était le lieu d'accueil et de charité ouvert à tous, et beaucoup y mouraient... faute de moyens thérapeutiques. Mais, en se technicisant à outrance, l'hôpital s'est déshumanisé. Odette Thibault le montre, illustrant son propos de quelques témoignages vécus. L'hôpital de demain, cependant, devra accueillir le mourant, et lui assurer la meilleure qualité de la fin de sa vie. Odette Thibault réclame, pour l'individu, le droit d'exprimer ses volontés concernant sa propre mort, de la gérer avec l'aide du médecin et, au besoin, de la choisir, au-delà des thérapeutiques, lorsqu'elles échouent. Entre l'abandon et l'euthanasie, il existe une troisième voie : les soins palliatifs. On peut les définir comme tout ce qui reste à faire lorsqu'on croit qu'il n'y a plus rien à faire, en commençant par le soulagement des souffrances, et en continuant par le "nursing" indispensable, et l'accompagnement psychologique du mourant dans son dernier voyage. L'auteur montre que de telles unités de soins palliatifs existent depuis longtemps à l'étranger, et peuvent servir de références à celles qui commencent à apparaître en France. On s'en occupe même au Conseil de l'Europe, au niveau théorique, mais il serait urgent de passer à la pratique. Pour changer la mort, changeons l'hôpital ! Mais, changer l'hôpital suppose une réforme profonde des mentalités, non seulement des médecins, des équipes soignantes, mais aussi des citoyens, dans une société qui doit intégrer la mort. Les révolutions qui se sont produites au cours de ces dernières années, dans le domaine de la maîtrise de la vie en général - dont la mort fait partie - exigent une révision au niveau éthique, et doivent déboucher - avec bien d'autres problèmes - sur un nouvel humanisme... qui reste à inventer.
