Comment accéder à un médecin la nuit, le week-end, ou les jours fériés ? L’organisation de la permanence des soins ambulatoires (PDSA) a pour mission de garantir l’accès aux soins, en dehors des urgences, durant ces plages horaires. La PDSA est assurée par les médecins exerçant en cabinet privé, dans les associations de type « SOS médecins » ainsi que dans les établissements de santé dans certaines conditions. Des services de garde sont également assurés par les pharmacies et les transports sanitaires. Avant 2002, les médecins libéraux avaient une obligation déontologique individuelle à pratiquer des gardes. À la suite d’une grève de ces gardes en 2002, les pouvoirs publics ont pris la décision de fonder l’organisation de la PDSA sur le volontariat accompagné d’une rémunération sous forme de forfaits d’astreinte. A cela s’est ajoutée une régulation téléphonique, via les centres 15 ou les associations de praticiens constituées à cet effet, afin d’apporter la meilleure réponse médicale possible, depuis le simple conseil jusqu’à la consultation. Les Agences régionales de santé (ARS) mises en place en 2010 ont pour mission d’encadrer l’organisation des gardes, en coordination avec les différents acteurs, définissant les secteurs géographiques et les cahiers des charges. Malgré toutes ces réformes et les efforts engagés par l’État, avec notamment une augmentation importante des dépenses en raison de la rémunération des astreintes, des difficultés persistent : une baisse continue du nombre de médecins volontaires, des inégalités territoriales, un recours aux urgences parfois inapproprié, un manque de connaissance du dispositif par le public, et une prise en charge déficiente concernant des publics spécifiques et fragiles (les personnes détenues ou gardées à vue par exemple). Partant de ces constats, la mission a adopté 16 propositions qui ne constituent pas le « grand soir » de la permanence des soins, mais s’appuient sur le pragmatisme et la nécessité de faire travailler l’ensemble des acteurs du système de santé afin d’apporter la réponse médicale la plus adaptée pour chaque patient durant les heures de fermeture des cabinets médicaux.
