Certes, les pouvoirs publics des trois pays maghrébins1 ont investi en faveur de la santé, ce qui a permis de faire face à certaines contraintes et d'améliorer notablement l'état de santé de la population, comme en témoignent la baisse de la mortalité infantile et l'accroissement de l'espérance de vie à la naissance, ainsi que l'élimination ou la forte réduction de l'incidence d'un certain nombre. [...] Selon cette approche, l'analyse de la gouvernance sanitaire consiste à vérifier le respect des principes de bonne gouvernance dans la réalisation des fonctions du système de santé (les fonctions du système de santé sont en général au nombre de quatre : l'identification des besoins et priorités, la production des soins et services, le financement de la santé et l'administration générale). [...] Par conséquent, la responsabilité de bonne gestion, d'amélioration continue de la qualité des services, de la disponibilité des moyens humains et financiers pour assurer la continuité et l'adaptabilité du service public de santé est à la charge directe des départements de santé. [...] De ce fait, pour avoir un réel impact sur la qualité de vie des populations et des résultats meilleurs dans le domaine de la santé, il faut agir sur tous les déterminants de la santé et renforcer la capacité des individus et des communautés à interagir avec l'ensemble des acteurs voués à contribuer à l'amélioration de leur qualité de vie. [...] La conjoncture montre que le continent supporte toujours le fardeau le plus lourd de la planète en termes de cas de VIH et de paludisme, et enregistre plus de la moitié des taux mondiaux d'incidence, de prévalence et de mortalité dus à ces maladies (Nations Unies 2014:77).

Most non-governmental organizations (NGOs) in the Arab world have traditionally been active in the areas of social work and charity, often within a religious or communal framework. But recently, many of these organizations have become the forum for conflicts between different political trends, while others tackle political problems such as human rights or democratic issues. Facing the rejuvenated NGO scene in the Arab world, public authorities remain torn between support for the concerns of civil society and the traditional mode of management, which does not delegate, consult, or decentralize. Can NGOs in the Arab world be considered full-fledged actors of governance and of national and local development? This report discusses several different approaches that support reforming health care services in developing countries. For some time now, health care services have been supported by government funds. As demands for improving health care services continue to increase additional demands will be placed on governments to respond. This, however, will not be easy. Slow economic growth and record budget deficits in the 1980's have forced reductions in public spending. Alternative approaches to finance health care services are needed. Such possible changes could involve: decentralization of federal government involvement; the promotion of nongovernment involvement; the imposition of user fees; and, establishing health insurance. Finally, the role of the Bank in pursuing new financing strategies is discussed.
