Abstract: Abstract Abstract of the thesis entitled A study on the role of probiotic Lactobacillus rhamnosus GG on gastric mucosal damages in rats Submitted by Lam Kai-Yee For the degree of Master of Philosophy at the University of Hong Kong In August 2006 The gastrointestinal (GI) tract is colonized by a vast community of microorganisms and maintenance of the microbial balance is essential for gut health. For decades, probiotics have been shown to maintain the GI equilibrium of indigenous microflora. Although numerous studies have advocated their use for the prevention and treatment of various GI disorders, the role of probiotics in gastric mucosal damage has been comparatively neglected. The aim of this study is to investigate the beneficial effects of a probiotic strain Lactobacillus rhamnosus GG (LGG) on acetic acid-induced gastric ulcer and ethanol-induced gastric lesions and to elucidate the mechanisms involved. The study found that LGG significantly and dose-dependently reduced gastric ulcer area. The cell apoptosis to cell proliferation ratio was strongly decreased and accompanied by significant upregulation of ornithine decarboxylase (ODC) and B-cell lymphoma 2 (Bcl-2) protein expressions at the ulcer margin. Angiogenesis was also significantly stimulated together with the induction of vascular endothelial growth factor (VEGF) and cyclooxygenase-2 (COX-2) expression. Furthermore, LGG up-regulated the phosphorylation levels of epidermal growth factor receptor (EGFR) and the downstream extracelluar signal-regulated kinase 1/2 (Erk1/2) iwithout altering total EGFR and Erk1/2 expression. These findings suggest that LGG enhanced gastric ulcer healing via the attenuation of cell apoptosis to cell proliferation ratio and increase in angiogenesis. Regulators of these processes such as ODC, Bcl-2, VEGF, COX-2, EGFR and Erk1/2 are likely to be involved in the ulcer healing action of LGG. LGG pretreatment also significantly reduced ethanol-induced gastric lesions in a dose-dependent manner. This was accompanied by the increase of basal mucosal prostaglandin E (PGE ) level and endothelial nitric oxide synthase (eNOS) protein 2 2 expression. In addition, LGG attenuated the suppressive action of ethanol on mucus layer, mucin 6 (muc6) mRNA expression and transmucosal resistance, and reversed the stimulatory action of ethanol on cellular apoptosis in the gastric mucosa. It is suggested that the protective role of LGG on ethanol-induced gastric mucosal lesions is likely attributable to the upregulation of PGE and eNOS, which in turn, stimulates the mucus secretion to increase the transmucosal resistance and subsequently protects cells from apoptosis in the gastric mucosa. Lactobacillus rhamnosus GG (LGG) is one of the best studied probiotic organisms. The ability of probiotics to adhere to other microorganisms and the intestinal epithelium is thought to play a major role in their protective functions. Coaggregation is an important mechanism for biofilm formation by microorganisms. The goals of this study were to examine the ways in which nutrient variation affects intercellular interactions between LGG and other gut microbes and the hydrophobic character of LGG. We hypothesized that nutritional variation may affect the ability of LGG to coaggregate and form biofilms and thus affect its probiotic characteristics and ability to colonize the gastrointestinal tract. Lactobacil/us rhamnosus GG was cultured anaerobically in different formulations of tryptone, yeast extract, and glucose (TYG) medium in order to simulate a milk-based, plant-based, and meat-based diet. Since the gut is mainly colonized by microorganisms belonging to the Firmicutes and Bacteroidetes phyla, we tested the coaggregation ability between LGG and Bacteroides spp. and ParabacJeroides spp. Our results suggest that coaggregation varies by media type, with LGG cultured in BeYG [Beef extract (meat protein), yeast extract, and glucose] medium exhibiting the highest coaggregation scores. Hydrophobicity values of LGG cells cultured in varying nutrient conditions differed significantly by media type, and LGG cultured in BeYG exhibited the highest hydrophobicity scores. These results indicate that different nutrient conditions may enhance the ability of LGG to colonize the human gut microbiome and enhance its ability to act as a successful probiotic.

The topical use of probiotics may offer new alternatives that are efficient and safe to use. L. rhamnosus GG (LGG) is one of the best studied probiotics. The aim of the study was to assess the effectiveness of both viable LGG and lysate in protecting human epidermal keratinocytes from the toxic effect of wound pathogens such as: Staphylococcus aureus, Pseudomonas aeruginosa, Escherichia coli, Streptococcus pyogenes and Acinetobacter baumannii. Trypan blue exclusion assay was used to assess the viability of human epidermal keratinocyte after 24 h of incubation with each wound pathogen, and following co-incubation of wound pathogens with viable LGG or with different concentrations of LGG lysate. All the tested pathogens significantly reduced the percentage viability of keratinocytes. Both viable LGG and its lysate significantly increased the viability of keratinocytes when co-incubated with S. aureus and E. coli. However, neither viable LGG nor its lysate induced significant increase in keratinocyte viability when co-incubated with P. aeruginosa, S. pyogenes and A. baumannii. In conclusion, both viable LGG and its lysate may have utility in prevention of wound infection. However, such effects may be restricted to certain pathogens.

Coaggregation has been indicated as a key mechanism in the formation of biofilms. This research sought to characterize the interactions occurring between native gastrointestinal Bacteroides & Parabacteroides and the probiotic strain Lactobacillus rhamnosus GG (LGG) cultured in Todd Hewitt (TH), deMan, Rogosa, and Sharpe (MRS), and Brain Heart Infusion (BHI) using in vitro coaggregation assays. In the coaggregation survey of interactions, a trend of growth medium-dependent coaggregation variability was displayed with LGG grown in TH displaying the widest spectrum of coaggregation with Bacteroides/Parabacteroides strains and narrower spectrum from the other cultures of LGG. By protease inhibition, it was confirmed that the presence of novel adhesin(s) occurs on LGG, mediating coaggregation with moderate strength to a variety of Bacteroides & Parabacteroides strains abundant in the large intestine, including selective interactions with capsule-deficient mutants of B. thetaiotaomicron VPI-5482. In the case of LGG grown in MRS, bimodal adhesin interaction with involvement of Bacteroides/Parabacteroides partners was observed. These interactions were not reversed by the addition of a variety of simple sugars, providing no support for the presence of a lectin-like adhesion on LGG, and thus the affinity of the adhesin(s) involved remains to be characterized.
