Due to the armamentarium of agents available to effectively suppress the immune system, the past decade has seen a shift in focus from prevention of rejection to a focus on extending the life of the allograft and novel strategies to increase the organ donor pool. A successful renal transplant for patients with kidney failure reduces mortality rate when compared to patients who continue dialysis. Organ donation from living donors has significant better results over organ donation from deceased donors. Traditionally the surgical approach for living donor nephrectomy was performed through a subcostal lateral incision. Now, laparoscopic donor nephrectomy has become the method of choice to procure kidneys from living donors, because of the reduced procedure-related morbidity, shorter convalescence period, better cosmetic result and a superior quality of life for the donor compared with open surgery. However, there are concerns about the transient function deterioration of the donor graft, compared to the traditional open procedure. The causes and exact mechanisms of the altered hemodynamic parameters (increase in systemic vascular resistance and a decrease in cardiac output), renal blood flow (a decrease up to 75%, with ! subsequent temporary renal ischemia) and reduced urine output during LDN, and delayed graft function after LDN are probably multifactorial. Various mechanisms, such as venous compression caused by elevated IAP, and the pharmacological action of the absorbed CO2, as well as activation of the sympathetic and neuro-humoral factors have all been suggested to explain these transient adverse renal effects. Therefore, it has been advocated to use vigorous hydration up to 2 l/h of crystalloids during LDN in patients. However, it was shown that this vigorous hydration could not prevent the impaired creatinine clearance as observed after LDN. Living donation is still needed to overcome organ shortage. All countries seem to increase and encourage such kind of donation according to medical and ethical guidelines. The results of renal transplantation from living donors are better compared to those from cadaveric kidneys. Since the first successful kidney transplantation from a living donor, some 63 years ago, surgery has shifted toward a less invasive approach offering to the donor less pain, better cosmesis, a shorter hospital stay, and a quick return to normal activities. Laparoscopic living-donor nephrectomy (LLDN) is now considered as the gold standard approach for kidney retrieval on live donors and has undoubtedly revolutionized kidney donation. It must offer to the donor safety, low morbidity, and fast recovery and must obtain a graft with adequate vessel length, short warm ischemia time, and well-preserved ureteral blood supply. We describe our technique of LLDN according to safety principles and reproducible steps. Highly qualified and well-trained surgeons are allowed to perform such techniques within a very well-equipped environment and with experienced surgical staff. A living donor program should undertake at least 30 cases per year to maintain adequate experience and offer less complication rate.

Morbidity in this group of patients is undesirable. However, the reported incidence of significant long-term surgical complications in this cohort, including incisional hernia (IH), remains variable. This study aimed to investigate the incidence and potential risk factors in the IH development post HALDN.Methods:A retrospective analysis of a contemporaneously maintained database and electronic records of patients who underwent intraperitoneal HALDN at a single centre over 10 years (01/2008-11/2018) was performed. Data collected included incidence and time to diagnosis of incisional hernia. Potential patient confounders (demographics, BMI, smoking status, respiratory co-morbidities, previous abdominopelvic surgery, hernia history) and surgical risk factors (hand port site, wound complications, re-operation, post-operative chest infection) were also assessed. Univariate and multivariate logistic regression analysis was performed.Results: There was an IH incidence of 8.9% (n=69) in 771 patients following intraperitoneal HALDN. Median time to hernia diagnosis was 12 months (IQR 5-16). Univariate analysis revealed respiratory co-morbidities (14.5% vs 6%, p=0.019), obesity (BMIu2265 30kg/m2) (35.5% vs 19.7%, p=0.017), wound complications (24.6% vs 9%, p=0.01), re-operation (7.2% vs 2.1%, p=0.027), post-operative chest infection (23.2% vs 10%, p=0.02) as risk factors for incisional hernia occurrence. On multivariate analysis respiratory co-morbidities (OR 2.96, 95% CI 1.21 u2013 7.24), re-operation (OR 3.64, 95% CI 1.04-12.69) and post-operative chest infection (OR 2.46, 95% CI 1.21 u2013 5.02) were significant predictive factors for hernia formation. Discussion:Incisional hernia post HALDN is a major complication with significant morbidity in a previously healthy population. Pre- and post-operative respiratory compromise and re-operation appears to increase risk. Focused prehabilitation and post-operative care (enhanced recovery) may therefore mitigate IH risk. Further studies comparing incisional hernias in total laparoscopic and laparoscopic assisted techniques are also needed to establish if technique refinement may be of benefit.
