En 1926, dans Inhibition, symptôme et angoisse, Freud propose une nouvelle théorie de l'angoisse dans laquelle le Moi joue un rôle capital. Dans ce nouveau cadre, le Moi fait plus que ressentir l'angoisse, il la produit. Cette théorie s'oppose à celle qui la précédait où l'angoisse était une transformation, dans certaines conditions, de la libido. On peut opposer ces deux théories en disant que l'une, la plus ancienne, était pulsionnelle, alors que l'autre est topique. Il est bien entendu que la topique sur laquelle se fonde cette dernière théorie, est la nouvelle topique introduite pour l'essentiel après 1920. Mais celle-ci ne peut se concevoir, selon l'auteur, que fondée sur la dernière théorie freudienne des pulsions introduite, elle aussi, après 1920 et ceci, en dépit du fait que certains de ses éléments sont apparus avant cette date. En fonction de quoi, l'auteur pense qu'il faut ajouter une troisième étape théorique où l'angoisse est bien produite par le Moi (la théorie topique), mais où le danger qui la suscite, ainsi que sa production même dans le Moi, ont des sources pulsionnelles. Cette troisième étape a, selon l'auteur, valeur de synthèse des deux autres. Ainsi, s'ouvre la possibilité d'un renouvellement théorique-métapsychologique de l'angoisse, sans pour cela changer la phénoménologie de celle-ci, surtout son rôle dans la névrose et dans la psychopathologie en général. De cette façon est satisfaite, pour ce cas, une exigence métapsychologique fondamentale, à savoir montrer la genèse des phénomènes psychiques, surtout les plus importants, à partir des pulsions, c'est-à-dire à partir des sources de la vie psychique.

Angoisse réelle, angoisse névrotique, angoisse morale... les trois principales sortes d'angoisse trouvent toutes leur siège dans le moi. C'est en 1926, après avoir formalisé avec la 2e topique «Le ça, le moi et le surmoi», que Freud propose, dans Inhibition, Symptôme et Angoisse, une nouvelle théorie de l'angoisse dans laquelle le moi joue un rôle capital. Il fait du moi le seul siège de l'angoisse : le moi fait plus que ressentir l'angoisse, il la produit. L'angoisse réelle, l'angoisse névrotique et l'angoisse morale se laissent rapporter aisément aux trois relations de dépendance du moi : le monde extérieur, le ça et le surmoi. Avec cette nouvelle conception, la fonction de l'angoisse comme signal indiquant une situation dangereuse est passée au premier plan. Cette conception s'oppose à l'ancienne où l'angoisse apparaissait comme une transformation de la libido. Benno Rosenberg propose, dans son ouvrage, une synthèse entre les deux théories de Freud : l'angoisse serait bien produite par le moi, mais le danger qui la suscite et sa production même dans le moi ont des sources pulsionnelles, dont le moi doit se défendre pour protéger son unité. Ainsi s'ouvre un renouvellement de la théorie de l'angoisse, surtout à propos de son rôle dans la névrose et dans la psychopathologie en général. La réédition d'un ouvrage devenu un classique.
