A travers l'analyse du fonctionnement d'une circonscription sanitaire du sud-Cameroun de la fin des années quatre-vingt, cet ouvrage décrit les transformations du champ médical sous l'effet de la crise. Il cherche à mieux situer, en ville et à la campagne, le rôle respectif de chaque protagoniste du système et notamment la part des pratiques traditionnelles à base de plantes, celle des rituels de contre-sorcellerie, celle des services apportés par les dispensaires et les hôpitaux, ou encore la fonction des pharmacies d'officines ou de celles dites " du panier ". Le lecteur comprendra mieux les succès des savoirs importés, favorisés par l'aspiration au progrès social (biomédecine, christianisme, pharmacie industrielle). Mais les anciennes approches thérapeutiques des mbmebela (faiseurs de remèdes) et des ngengan (contre-sorciers) influencent encore le champ médical moderne en favorisant des dérives : dispensation anarchique du médicament et dérives rituelles des Églises dites " éveillées ". Cette forme de " modernisation " ambiguë échappe au contrôle des appareils officiels et favorise la marchandisation des prestations sanitaires ainsi que les mésusages de la pharmacopée industrielle et traditionnelle. Sans l'effort d'un tel diagnostic des enjeux du marché de la santé, l'augmentation en dotations du système hospitalier actuel équivaut à subventionner les pratiques abusives tant contestées.
