A curious ambiguity surrounds errors in professional working contexts: they must be avoided in case they lead to adverse (and potentially disastrous) results, yet they also hold the key to improving our knowledge and procedures. A growing body of research has increased scholarly and managerial awareness of the enormous potential for organizations to learn from errors. This paper investigates the range of work contexts in which errors occur in organizations and the implications of this variation for organizational learning from error. By organizational learning from error, we refer to organizational activities that both build understanding of what went wrong to cause an error and identify ways to prevent the same or similar errors from occurring in the future. We suggest that different kinds of work give rise to different conditions of error, and these distinctions influence what organizational approach and actions are best to maximize potential learning from an error.

Most studies of error focus on its reduction or elimination. Clearly, there are many steps that can be taken to avoid or prevent the occurrence of errors. Yet in human systems, error is inevitable. This is commonly argued on the grounds that people can become tired, or confused or distracted, or fail to attend to their work, or are in some other way inherently fallible. All of these are real factors in many errors, of course, but for systems of cooperative work in the real world, there may be a less disparaging and more fundamental reason for the inevitability of error. This is that such systems always rely on learning on the job, and where there is the need for learning, there is potential for error. A naturally situated system of cooperative work must both produce the intended result of its process and reproduce itself at the same time. Such cooperative systems may change over time, be reorganized, change the things they do, and change the technology they utilize to do the job. Even if tasks and tools could be somehow frozen, changes in personnel are certain over time. Most commonly, relatively expert personnel are gradually lost while relatively expert personnel are added. Even if the skills required to do the job can be taught in schools, the interactions that are characteristic of cooperative work can generally only be learned on the job. (sdw).

This book discusses competence, teacher competence, and professional error competence of teachers, and emphasizes the need for a training programme that supports the latter. The book starts out by presenting results from previous studies that underline the necessity to train professional error competence of teachers, especially in the field of accounting. The studies analysed include research in the field of accounting, and on the efficacy of teacher training. Next, considerations on training programmes are presented. From these analyses, a training programme was designed to support professional error competence in accounting. This training programme aims for increased knowledge about students’ errors (content knowledge) and offers strategies to handle these errors (pedagogical content knowledge). Both are central facets of professional error competence. The book describes the development, characteristics, implementation, and evaluation of this programme. It details the test platform that was developed and used for the assessment of professional error competence, and critically discusses the results from the evaluation of the training programme from various perspectives. The current discussion on teacher training and expertise is influenced by empirical results obtained in international large-scale studies such as PISA and TIMSS. The findings of the studies underpin the discussion on teaching quality and teachers’ professional competences. The key issue is that teacher competence has an impact on teaching quality and this, in turn, influences students’ achievements. International comparative studies reveal that teachers often lack central competence facets, and therefore it is assumed that standard teacher training programmes may fail to successfully prepare student teachers for their tasks. Therefore, customized training programmes are currently being discussed. Their focus is mostly on pedagogical content knowledge and classroom practices, because these competence facets are essential for teaching quality.
