Contexte : Les risques de développer un trouble de l’usage des substances (TUS) sont augmentés si la consommation est débutée jeune. Les jeunes sont hypers connectés, l’utilisation du web en prévention dans cette population peut être intéressante. Objectif : L’objectif de cette étude est d’examiner l’étendue et la nature de l’activité de recherche sur les interventions de prévention primaire proposées sur internet afin de prévenir les troubles de l’usage des substances chez les jeunes de 11 à 25 ans. Méthode : La recherche a été menée selon les critères PRISMA-Scr. Les bases de données Pubmed et Cochrane librairy ont été exploitées avec les mots-clés suivants : ("primary prevention" OR "prevention" OR "substance abuse prevention" OR "substance use disorder prevention") AND ("Substance use" OR "smoking" OR "substance use disorder" OR "addiction" OR "alcohol" OR "cannabis" OR "drug use disorders" OR "tobacco") AND ("adolescent" OR "youth" OR "teenager") AND ("internet-based program" OR "social media" OR "facebook" OR "twitter" OR "snapchat" OR "youtube" OR "instagram" OR "web-based"). La période évaluée était 2009 à 2019. Les études incluses évaluaient l’efficacité des interventions pour réduire les consommations, les facteurs de risque de développer un TUS ou pour augmenter les facteurs protecteurs. Résultats : Au total 418 articles répondaient à ces critères de recherche, 32 articles étaient inclus après lecture des études, et 30 articles ajoutés d’après les références des articles sélectionnés. 82.3% des études décrivaient une intervention de prévention ayant une efficacité positive sur les consommations de substances ou les facteurs de risque et de protection des TUS. Les interventions étaient diffusées dans le cadre scolaire ou dans l’environnement socio-familial des jeunes. Discussion : Ces actions pouvaient avoir un effet sur les facteurs individuels de vulnérabilité et de résistance comme les compétences psychosociales et l’âge de début des consommations ou les facteurs environnementaux comme la dynamique familiale et influence des pairs. Près de la moitié des campagnes de prévention sur internet étaient menées dans le cadre scolaire. Les études proposant des interventions dans l’environnement socio-familial étaient prometteuses. La majorité des interventions concernaient l’alcool. Des recherches pourraient approfondir l’effet des interventions sur le web visant les interactions familiales et s’étendre aux autres substances que l’alcool, ainsi que l’effet de l’usage des réseaux sociaux comme vecteurs de messages de prévention sur internet. Conclusions : Ce type d’intervention peut s’inscrire comme support supplémentaire dans le cadre global de la prévention primaire.
