Au-delà de la cancérologie, les soins palliatifs se sont étendus à l'ensemble des maladies chroniques évolutives mettant en jeu le pronostic vital. Les études démographiques montrent que la population française vieillit inéluctablement et que les fins de vie se concentreront à des âges de plus en plus avancés. La littérature met en avant les spécificités de la prise en charge palliative des personnes âgées. Entre le 1er et le 30 juin 2009, nous avons mené une enquête rétrospective auprès de patients âgés relevant de soins palliatifs, au sein de 2 structures (Lits Identifiés Soins Palliatif [i.e. Palliatifs] du centre de gérontologie Balmès et Unité de Soins Palliatifs et d'Accompagnement du CHU St-Eloi). Une même grille d'identification a été utilisée pour les 2 populations. Les buts de l'étude étaient de faire un état des lieux des patients âgés relevant de SP sur le CHRU de Montpellier, de mettre en évidence ces spécificités puis de tenter d'évaluer un outil d'identification. Les spécificités des soins palliatifs gériatriques étaient encore plus mises en évidence en comparant la population de Balmès donc initialement gériatrique, avec celle de l'UASP donc directement en phase palliative. Sur le plan clinique, ces patients ont une diversité de pathologies cumulées, sont plus dépendants. L'expression et l'évaluation de leurs besoins sont limitées. Sur le plan psycho-relationnel, ces patients sont demandeurs de temps, d'écoute et surtout de considération. Sur le plan socio-familial, prendre en charge ces patients peut aboutir à des situations difficiles si l'on n'est pas soutenu ou formé. Une grille d'identification des patients âgés relevant de soins palliatifs s'avère utile voire nécessaire pour éviter l'abandon thérapeutique ou l'obstination déraisonnable mais nullement suffisante pour assurer des soins de qualité. La réflexion éthique reste de fait indispensable. Les soins palliatifs gériatriques sont donc un enjeu de santé publique. La réponse ne viendrait-elle pas alors du développement d'unités spécifiques?
