In Japan, cadaveric donor liver transplantation is not common even though cadaveric organ transplantation was legally established in 1998. In contrast, the number of living donor liver transplantations is increasing, with more than 1700 cases at 43 Japanese institutes by November 2001. Indications for and have become living donor liver transplantation are widening in Japan similar to those for cadaveric donor liver transplantation in the United States and Europe. At the same time, split liver transplantation from cadaveric donors shares some technical aspects with living donor liver transplantation. Remarkable progress has been reported recently, and thus it was an auspicious time to hold a symposium on "Current issues in liver/small bowel transplantation" in Japan. We were honored to hold a very fruitful symposium sponsored by the Keio University Medical Science Fund and to bring together top-rank transplant surgeons from Japan and other countries. It was a productive and rewarding time for all participants. We were able to share our experience through excellent presentations followed by active discussions and insightful com ments. At the symposium, we focused on current issues in liver transplanta tion such as widening indications for viral hepatitis and malignant tumors. We also discussed technical aspects and physiological problems in split/iiving donor liver transplant, novel strategies in immunosuppression, and the current status and future prospects in small bowel transplantation. (Uncorrected OCR) Psychosocial Outcomes 1 Abstract Living-donor liver transplantation (LDLT) is performed increasingly frequently in Hong Kong. The goal of this study is to explore the psychosocial outcomes and donation motives of living donors from their perspectives. Donors were interviewed A retrospectively using a locally constructed evaluation instrument, which explored pre-donation motives and concerns and also assessed multiple dimensions of the liver transplant donors|psychosocial well-being and adaptation after transplantation surgery. The Medical Outcomes Study 12-Item Short-Form Survey Version 1 f (SF-12v1) was used to measure global health-related quality of life. Eighty donors were recruited to participate in the semi-structured interview. No long-term physical complications from the surgery were reported. Ninety-eight percent of donors resumed their pre-donation job at one year post-donation. All donors had resumed pre-donation social activity and usual lifestyle at the time of the interview. Some donors perceived a slight declined in health (25%), work ability (12.5%) and in handling daily life functions and activities (15%). The most common pre-donation motive was to save the life of beloved ones. Despite that 56% of the donors reported having recurrent mild physical discomfort over their surgical wound, none of the donors expressed any regrets concerning their decision and participation in the recipient| surgery and would opt to donate again if need to. The available results are Psychosocial Outcomes 2 the first of its kind in Hong Kong, and add to the literature on our understanding of the personal costs and benefits of living-related donor liver transplantation. The available data also serves as useful information of reference for future potential donors.
