Longitudinal access to disability benefits is affected by interactions in benefit eligibility between the Disability Insurance (DI) and Supplemental Security Income (SSI) programs and lags arising from processing time in receiving the first payment. Administrative records show that a quarter of the calendar year-2000 cohort of first-ever working-age disability awardees were involved with both programs over a 60-month period, indicating a higher degree of program interaction than apparent from cross-sectional data. Nonbeneficiary status is three times more prevalent 60 months after entry among those who entered SSI first compared with DI entrants, as a result of exits that are due to the SSI means test. Over half of new awardees qualifying for both DI and SSI benefits are eligible for SSI during 4 or 5 months of the 5-month DI waiting period, but many do not receive their first SSI payment until later because of lags in final award decisions.

We follow six cohorts of childhood Supplemental Security Income (SSI) disability awardees for a time horizon up to 30 years, using program records on demographics, type of impairment, SSI and Disability Insurance (DI) recipiency, and mortality. We use descriptive analysis and multinomial logit regression for repeated cross-sections of the six award cohorts, controlling for years since first award. For all award cohorts, many individuals transition from SSI recipiency to DI or nonbeneficiary status. Others die over time. Accounting for DI program participation is necessary to obtain a full picture of disability program participation in adulthood. SSI-only recipiency substantially diminishes in adulthood. However, DI involvement increases. An increasing proportion of individuals receives both benefit types (SSI and DI) as the cohorts age in adulthood. The trajectories of outcomes across successive award cohorts change in important ways. First, we observe a strong trend of increased transitions to nonbeneficiary status among survivors as we move from early award cohorts to later cohorts, with a sharp upward shift around the time of welfare reform in the mid-1990s. Second, the data show a secular decline in mortality across award cohorts. The data suggest that a substantial portion, but not all, of that decline has been affected by the Supreme Court's Zebley decision. Increased incidence of transitions to nonbeneficiary status and reduced mortality across award cohorts have opposing effects on the duration of disability benefit receipt.

This article explores the role of the Social Security Disability Insurance (DI) and Supplemental Security Income (SSI) cash benefit programs in providing access to public health insurance coverage among working-aged people with disabilities, using a sample of administrative records spanning 84 months. We find that complex longitudinal interactions between DI and SSI eligibility determine access to and timing of Medicare and Medicaid coverage. SSI plays an important role in providing a pathway to Medicaid coverage for many low-income individuals during the 29-month combined DI and Medicare waiting periods, when Medicare coverage is not available. After Medicare eligibility kicks in, public health insurance coverage is virtually complete among awardees with some DI involvement. Medicaid coverage continues at or above 90 percent after 2 years for SSI-only awardees. Many people who exit SSI retain their Medicaid coverage, but the gap in coverage between stayers and those who leave SSI increases over time.

Over three-fourths of the working-age population in the United States is insured for Disability Insurance, which protects against the decline in earnings associated with severe disability. However, little is known about the various ways the Supplemental Security Income (SSI) program provides additional protection. Using data from the Survey of Income and Program Participation, we find that over one-third of the working-age population is covered by SSI in the event of a severe disability. Three important implications follow: (1) SSI increases the coverage of the working-age population; (2) SSI enhances the bundle of benefits available to disabled individuals; and (3) SSI enhances the safety net by interacting with other programs, most notably by increasing health insurance coverage because it provides an important path to Medicaid coverage. Ignoring these implications could lead to inaccurate inferences about disability program coverage, health insurance coverage, and the well-being of working-aged individuals with disabilities.

This paper addresses impairment co-morbidity among participants in programs that provide dis-ability benefits. Co-morbidity in this context is defined as the simultaneous presence of a primary and a secondary medical diagnosis. I fit a high-dimensional Bayesian multivariate probit model with a 10% random sample of 2009 initial claimants (disabled workers, including individuals concurrently applying for Disability Insurance and Supplemental Security Income). The resulting correlation estimates provide evidence of strong impairment co-morbidity patterns at the initial-claim level. Many of the findings mirror the epidemiological evidence, such as associations of diabetes with chronic renal failure, open wounds of a lower limb, peripheral neuropathies, and blindness/low vision. Other results are surprising. For instance, the correlation estimates defy the presumption of high positive association between mental and musculoskeletal-system diagnoses.
