Comprised exclusively of nearly two dozen clinical cases covering fractures of the tibia, this concise, practical casebook will provide orthopedic surgeons with the best real-world strategies to properly manage injuries to the tibial shaft, plateau and pilon, as well as deformities, nonunions and bone loss. Each chapter is a case that opens with a unique clinical presentation, followed by a description of the diagnosis, assessment and management techniques used to treat it, as well as the case outcome and clinical pearls. Cases included illustrate different management strategies for Schatzker (I-VI) tibial plateau fractures, plates and screws for proximal tibia fracture, intramedullary nailing for midshaft and distal tibial fracture, and the use of Ex-Fix with open tibia and distal pilon fracture, in addition to Masquelet bone grafting and modified clamshell osteotomy for acute shaft fracture. Pragmatic and reader-friendly, Fracture of the Tibia: A Clinical Casebook will be an excellent resource for orthopedic surgeons confronted with various injuries to the shin.

Purpose: To compare the results of institutional protocols at two high-volume trauma centers with regard to the treatment of soft tissues in the setting of open tibial shaft fractures. Methods: This is a retrospective review at two high-volume Level 1 trauma centers of all open tibial shaft fractures (OTA 42) identified using a prospectively and retrospectively collected fracture database over a 6 year period. Providers at Site 1 advocate for acute primary closure while those at Site 2 encourage repeat debridement and delayed coverage for wounds not easily closed. Patient demographics, injury and treatment characteristics were recorded. Primary outcome was 90-day wound complication including superficial infection, deep infection and wound dehiscence. Secondary outcomes included reoperation, infection, non-union and amputation rates. Results: 219 patients were included at Site 1 and 282 at Site 2. Cohorts were similar with regard to patient demographics and comorbidities. Site 1 closed significantly more traumatic wounds at index surgery (78% v. 36%, p

"The overall aim of this thesis is to contribute to individualizing the management of patients treated with IMN for a TSF in order to improve patient care after personal risk assessment (i.e. risk stratification). To achieve this, a more in-depth knowledge is required in regard to the various complications and subsequent surgical procedures that can occur. Particularly, a better insight into patient specific risks is essential to further individualize the management of patients with tibial shaft fractures , especially in our era of personalized medicine and data driven care. This may lead to an improvement of patient care and a prevention and reduction of complications. This thesis will consist of three parts. In Part I we aim to identify patient specific risks of subsequent surgery in order to individualize patient consent and anticipate peri-operative management. In Part II we aim to determine patient specific risks of complications in order to individualize diagnostic work-up and peri-operative treatment plans. In Part III we aim to define to what extent iatrogenic complications limit patients’ functional performance, and to what extent these complications can be accepted to guide (post) operative management."--
