This book was originally conceived in 1987. It was then seen as a contribution towards improved management and policy-making in a diffuse and neglected area of NHS management. The focus of the book is the 'old' Community Health Services: those transferred to Area Health Authorities from local authorities in the 1974 re organization of the NHS. These diverse services, while grouped together, had little objectively in common, occupying, as they do, a hazy middle ground between hospital and Family Practitioner Services. However, since 1974 there have been a number of major devel opments which have opened opportunities for change and devel opment in these services. These include: the resurrection of concern with 'Public Health'; the attempted closure of large mental illness and mental handicap hospitals and the development of 'Com munity Care'; the introduction of General Management; and the implication for health and local authorities of the White Papers 'Caring for People', 'Promoting Better Health' and 'Working for Patients'. Traditionally, Community Health Services were seen as low status and a professional dead-end. This, in tum, has led to a rather uneven body of literature. The growth of general management has led to a demand for a more coherent, management-orientated literature. It is our hope that this book will encourage the production of more literature in this area.

Managing Madness in the Community dispels the myth. The point of departure for Managing to Care is widespread concern that the present delivery of health and social welfare services is fragmented, uncoordinated, inefficient, costly, wasteful, and ultimately detrimental to clients' health and wellbeing. Dill traces the evolution of case management from its start as a tool for integrating services on the level of the individual client to its current role as a force behind the most significant trends in health care. Those trends include the entrenchment of bureaucracy, the challenges of once dominant professions, and the rise of corporate control. The author's purpose in adopting this analysis is to invite further scrutiny of the case management profession, and at the same time to identify new possibilities for its application.This volume brings together thoughts developed over many years of observing and participating in case management programs. Despite the tremendous progress and successes achieved in diagnostics, therapy, and rehabilitation in psychiatry over the past few decades, the stigma towards mental health patients, their relatives and caregivers, and healthcare professionals is still present. Social stigma, in particular, represents a major obstacle to maintaining adequate mental health care. This increases reluctance to seek help delays patients' diagnosis and limits their compliance and adherence to treatment. In the long term, this reduces psychiatric rehabilitation effectiveness and causes a burden to healthcare providers and society alike.
