In this way we seek to contribute to as weIl as to stimulate the search for rational solutions to surgical problems. It is assumed throughout that the reader is fa miliar with the technical fundamentals of internal fixation, and so these details are omitted. Instead, special indications and technical refinements are presented on the basis of case examples. Because an endless variety of situations can arise in orthopedic surgery (a circumstance that is attracting more and more surgeons to the field), we have taken ca re that our examples can readily be applied to novel situations. With a great appreciation of history, Heinz Sarkowski has impressively reconstructed the company cor- spondence, which is fortunately almost completely preserved, and made it speak. * There is an inexhaustible amount of c- respondence pertaining to the period I have taken it upon myself to cover, and working through it properly not only would have required many years, but also would have detracted from the immediacy of the account. Thus, I decided to proceed from personal experience, to describe what has happened and to provide details gleaned from the correspondence. I have - counted here by no means only my own, but rather the personal experiences of the many company members and employees who are mentioned below. With the founding of the New York firm, developments branch out, becoming parallel but separate, and the change from one scene to another repeatedly interrupts the continuing course of events and the chronological flow of the report. In this connection, the occasional repetition of certain facts was - avoidable. In some places, however, it seemed more appropriate not to interrupt particular lines of development, but to describe them in continuity without regard to specific periods of time.

The theme of this work is the application of the engmeermg theory of frictional torque to total hip replacement. The author adhered tenaciously to this theory, involving the use of a small-diameter femoral head, throughout the epoch when the large-diame ter, metal-to-metal design dominated the field. During that considerable period general satisfaction with the early results rendered criticisms of the large-diameter head unwelcome. There was a formidable array of counter criticism: the small head would pierce a film of synovial fluid; the small head would wear the socket too rapidly; the small head would always have a high risk of dislocation; detachment of the trochanter, to achieve precise orientation for the small head, was unacceptable. But all these objections have now been largely overcome. Lubrication of high molecular weight polyethylene (HMWP) on metal is now accepted as being mainly by the boundary regime with thick fluid films playing no part. We now know that HMWP can indeed tolerate the very high stresses imposed by the small head and in tribological theory there may even be some advantage in high stress. Dislocation is now known not to be an automatic sequel to the small head.

In 1921 the Swiss surgeon BIRCHER published the first report on ar throscopy of the knee joint. The initial trials with the method revealed problems, mainly technical in origin, which caused it to be abandoned and forgotten until recently. Modern technical improvements, for which Japanese orthopaedic surgeons, above all WATANABE, were mainly responsible, have now led to a revival of interest in this method of investigation. The quality and versatility of the instruments currently available and the experience of the endoscopists who use them are such that arthroscopy of the knee joint is becoming an indispensable diagnostic aid. Arthroscopy is superior to arthrotomy in every respect; it is not only simpler but can be carried out on an ambulatory basis under local anaesthesia. In addition, it provides more information than arthrotomy, and the arthroscopic findings can be simply documented photographically. The complication rate is extremely low; cumulated statistics recently obtained from six clinics revealed no case of infection following nearly 4,000 arthroscopies. The main indication for arthros copy is the "painful, diagnostically obscure knee joint" in the widest sense. The procedure is also useful for further investigation of knee joint lesions which have already been diagnosed, as it allows their type and extent to be diagnosed more precisely, and the indication for surgery in a given case can be assessed more clearly. Unnecessary surgery can often be a voided in this manner. Arthroscopic biopsy is simple, and a variety of operations (e. g.
