This dissertation studies the entanglement of the history of médicos titulares, doctors responsible for providing home health care for the poor and fulfilling a number of public health and other official duties, with the political crisis of the Restoration state. Médicos titulares were affected by the political circumstances of the Restoration in a number of ways. Regulated by the state, but dependent upon municipal councils for their salaries, m©♭dicos titulares were at the mercy of caciques, political bosses who controlled the system of clientelism and patronage that linked local elites to national politics. Caciquismo made it difficult for these doctors to make a living and hindered both the implementation and reform of Spain's public health regime. The combination of caciquismo and resistance to the infringements by public health regulations upon municipal autonomy created a situation in which the public health regime on the ground varied greatly from the regulations on paper. Caciquismo and debates about local administration also impeded the ability of politicians to pass a comprehensive public health reform that would support the médicos titulares. The failure of Restoration politicians to pass a public health reform over the course of two decades promoted frustration and radicalism among médicos titulares and encouraged the group to join the majority of Spanish society in their accession to the 1923 pronunciamiento of General Miguel Primo de Rivera. The history of the médicos titulares bridges the two main historiographies on the Restoration era: studies of caciquismo and examinations of the Restoration crisis from a national perspective, by demonstrating how caciquismo helped erode the regime's legitimacy. The growing discontent and frustration of the médicos titulares with the Restoration state provides a case study of how one group of professionals experienced the crisis of liberalism and became willing to accept authoritarianism when the option was presented. The study also demonstrates the importance of examining both the legislative history and the implementation of public health regimes when assessing such regimes and contributes to the history of rural healthcare and poor relief in Europe.
