It would have been difficult at the beginning of the 80's to have predicted that by the end of the decade, Medical Virology would have become one of the most important topics in the area of both basic and clinical research. Although we were expecting a progressive increase in awareness of the role played by viruses in different diseases, we did not expect the outbreak of a fatal disease that was going to shake the roots of our society. The appearance of the human immunodeficiency virus (HIV-1) in the early 80's, has prompted a unique re search impetus in the area of Medical Virology. The knowledge that we are gaining in our attempt to understand the biology of HIV-1 and the immuno logical response to this virus should not only help us control the spread of this virus, but should also help us to better understand other viral infections. Let us hope that during the 1990's we can learn how to control HIV-1 infections so that by the end of the decade, no more human lives succumb to an infection with this virus. This book was written during a period when the technologies of genetic engineering were being applied to the study of animal viruses and when the organization and function of individual virus genes were being elucidated. This book, which uses human and animal viruses as models, aims to under stand the developments in molecular virology during the last 20 years. Al though molecular virology could also be taught by means of bacteriophages or plant viruses, the advantage of using animal viruses is in their ability to cause human and animal diseases as well as to transform cells, a primary problem in medicine. For the sake of clarity and convenience, not all the individual contributors to the various aspects of molecular virology were cited in the text. Instead, the reader is referred to review articles or key papers that list the numerous excel lent publications that have contributed to clarification of the various molecular processes. Thus the end-of-chapter bibliographies will guide the reader to the publications in which the original contributing authors are quoted. References given under the heading Recommended Reading are intended to assist those interested in pursuing a given subject further. I hope that this book will fulfill the purpose for which it is designed, and I urge readers to contact me if errors are found or updating is required.

Named for the enlarged, inclusion-bearing cells characteristic of infection by these viruses, cytomegaloviruses present a significant challenge to both microbiologist and immunologist. Although most primary infections in humans are subclinical, cytomegalovirus can be associated with a wide spectrum of disease, particularly when infection occurs in the immuno compromised individual or as a result of congenital or perinatal infection. Although reinfection with cytomegalovirus has been demonstrated, most recurrent and persistent infections result from the reactivation of latent virus. Cytomegaloviruses, like other members of the Herpesviridae family, have the capacity to establish latency after a primary infection but the mechanisms for establishing the nonreplicating but reactivat able state have not been defined. The factors responsible for the spectrum of manifestations of cytomegalovirus infection are largely undetermined but host immunological function, route of infection, and size of inoculum all contribute to the extent and severity of disease. Cytomegaloviruses have the largest genomes in the herpes virus family, approximately 240 kilo base pairs, providing a potential coding capacity for more than 200 proteins of which less than one-fourth have been mapped and described. There are many similarities to other herpes viruses in genome structure and gene expression; for example, three temporal classes of genes can be identified as rx (immediate-early), f3 (early), and y (late) products. The first five chapters of this volume review and describe recent developments in understanding the trans cription and regulation of these gene classes.

Viruses are studied either because they cause significant human, animal or plant disease or because they are useful materials for probing basic phenomena in biology, chemistry, genetics and/or molecular biology. Arenaviruses are unusually interesting in that they occupy both categories. Arenaviruses cause several human diseases known primarily as the hemorrhagic fevers occurring in South and Latin America (Bolivia: Machupo, Argentine, Junin virus, and Brazil: Sabia virus) and in Africa (Lassa fever virus). Because such viruses produce profound disabilities and often kill the persons they infect, they are a source of health concern and economic hardship in the countries where they are prevalent. Further, they provide new problems for healthcare persons owing to the narrowing of the world as visitors from many countries travel increasingly to and from endemic areas and may incubate the infectious agent taking it from an endemic area into an area where the virus is not expected. Such cases are now being re corded with increasing frequency. In addition to these hemor rhagic fever viruses, the arenavirus lymphocytic choriomeningitis virus (LCMV) can infect humans worldwide, although the illness is most often less disabling and severe than those elicited by the other arenaviruses. Yet, LCMV is of greater concern to non arenavirologists and experimentalists using tissue culture or ani mals, etc. , because normal-appearing cultured cells or tissues from animals used for research may be persistently infected with LCMV without manifesting clinical disease or cytopathology and may transmit that infection to laboratory workers.
