The current study examined the mental health profiles (current and lifetime) of adolescent girls and boys with serious behaviour problems. Age of onset, gender differences and comorbidity patterns were described. Youth in this study had substantial comorbidity, with approximately three quarters of youth with conduct disorder meeting criteria for at least two additional lifetime mental health disorders. Males and females had comparable rates of conduct disorder, ADHD, and substance dependence disorders; however, significantly more females than males met criteria for internalizing disorders (major depressive episode or PTSD). Age of first symptoms started at approximately 11-13 years of age in early adolescence. Approximately two thirds of the youth met criteria for childhood onset conduct disorder and one third met criteria for adolescent onset conduct disorder, with no gender differences. These findings highlight the need for early, comprehensive interventions for youth with serious behaviour disorders.

The current study examined the longitudinal mental health needs of males and females with serious behaviour problems and their experiences with services. Study One assessed the mental health profiles of youth in adolescence (N = 141; M = 15.28, SD = 1.42) and young adulthood (N = 69; M = 19.85, SD = 1.42). Rates of service use and barriers to care were examined quantitatively in young adulthood. Study Two further explored youths' experiences with services through an in-depth qualitative interview (N = 19; M = 23.56, SD = 1.52). Study One results demonstrated that mental health problems persisted from adolescence to young adulthood. Approximately half of participants endorsed clinically-elevated externalizing problems (substance dependence, antisocial behaviour, ADHD) and a third endorsed clinically-elevated internalizing problems (depressive and PTSD) in young adulthood. Externalizing problems appeared to be more stable than internalizing problems from adolescence to young adulthood. Despite this need, only 53% of participants in young adulthood accessed services and 43% reported at least one barrier to care. Study Two provided detailed accounts of youths' life histories, experiences with services and barriers to care. Participants reported experiencing severe child maltreatment, highlighting the need for early intervention services that protect youth from harm. Results pointed to the need for youth-centered services that are strengths-based, flexible, and use a harm-reduction approach. Youth preferred service providers who were empathic, patient, consistent and non-judgmental. The barriers to care themes were complex and occurred at the structural (e.g., lack of availability), familial (e.g., caregivers impacted their ability to access services), and individual (e.g., negative expectation of therapeutic relationship) levels. Findings are interpreted based on youths' histories of harmful interpersonal relationships and traumatic experiences. Clinical and policy recommendations are discussed.

Healthy mental, emotional, and behavioral (MEB) development is a critical foundation for a productive adulthood. Much is known about strategies to support families and communities in strengthening the MEB development of children and youth, by promoting healthy development and also by preventing and mitigating disorder, so that young people reach adulthood ready to thrive and contribute to society. Over the last decade, a growing body of research has significantly strengthened understanding of healthy MEB development and the factors that influence it, as well as how it can be fostered. Yet, the United States has not taken full advantage of this growing knowledge base. Ten years later, the nation still is not effectively mitigating risks for poor MEB health outcomes; these risks remain prevalent, and available data show no significant reductions in their prevalence. Fostering Healthy Mental, Emotional, and Behavioral Development in Children and Youth: A National Agenda examines the gap between current research and achievable national goals for the next ten years. In September 2012, the Substance Abuse and Mental Health Services Administration (SAMHSA), the Centers for Medicare & Medicaid Services (CMS) and the Administration on Children, Youth and Families (ACYF) held a technical expert panel to identify what the research tells us about services for children in therapeutic/treatment foster care (TFC)1 with behavioral health issues (see Appendix A for meeting agenda). The Life-Course of Serious and Violent Youth Grown Up addresses significant gaps in the literature on youth involved in chronic, serious, and violent offending. Through longitudinal research and a long follow-up into adulthood, it challenges common perceptions about offending outcomes. Using theoretically grounded, methodologically sophisticated and empirically driven research, this book culminates 20 years of data emerging from the Incarcerated Serious and Violent Young Offender Study (ISVYOS). Initiated in 1998 to understand the origins of serious and violent youth offending, it follows 1,719 formerly incarcerated youth through adulthood and offers a contemporary perspective to questions about chronic offending in adolescence and social and offending outcomes in adulthood. The authors provide a theoretically framed examination of new findings from the ISVYOS regarding participants’ justice system involvement, from onset to persistence to desistance. Most participants experienced continued involvement in the justice system in adulthood. However, contrary to past literature, ISVYOS findings challenge static descriptions of chronic offending and notions of the youth "super predator". ISVYOS findings also challenge assertions that experiences and risk factors in childhood and adolescence are not informative of adult justice system involvement. Together, the findings call for a more humanistic approach that recognizes that the complex lives of individuals formerly incarcerated in adolescence implies that desistance does not happen by default. This volume is the result of an EU project involving two different European countries (Italy and Cyprus) on risk and needs assessment for juvenile violent offenders.
