It affects significantly the quality of life and has important social and economic consequences (1). Complex regional pain syndrome (CRPS) is one of the most severe neuropathic pain conditions. Up to 26% of patients with surgical treatment of traumatic hand injuries may develop CRPS (2).Menthol has been used for analgesia since the ancient times. Recent interest on menthol in pain management has risen since 2002 when the menthol sensing channel transient receptor potential melastatin 8 (TRPM8) was identified (3,4). Since then menthol has been shown to be effective on the management of several acute nociceptive, inflammatory and neuropathic pain conditions.Here we present a case report where menthol was used in the treatment of refractory CRPS after traumatic hand injury and surgery.CaseA 39 years old non-smoking healthy male had a traumatic complete tear of the distal biceps tendon on 18th of June 2017. Two days later a surgery was performed to reattach the tendon to the bone. After surgery he developed a severe lancinating neuropathic pain at the elbow area. Pain was worse on night-time, NRS 7-8/10 and he could sleep only 3-4 h a night. Paracetamol-codeine and titzanidine were uneffective, gabapentin had some effect, but he was so somnolent at daytima that he could not return to work. Four weeks after the primary injury he was prescribed menthol-gel (Ice Poweru00ae, Fysioline Ltd., Tampere, Finland) for day-time used and menthol patches for nights on the elbow area. Since the first night, he was able to sleep 6-7 h a night and pain rating decreased to 3-5/10. After 4 weeks of treatment, he returned to work and was able to stop the menthol-use. Six months after the injury the muscle function has restored, he may have some muscle aches in few nights a week, which are relieved with menthol gel. At daytimes, he has no pain nor other symptoms.Discussion and conclusionsMenthol is a counter-irritant compound. Initially it stimulates nociceptors and then desensitize them. TRPM8 channels are expressed widely in pain system. In periphery 15% of sensory neurons are TRPM8 activated by menthol. Topically applied menthol activates higher central analgesic pathways also. TRPM8 receptors are expressed in the dorsal root ganglia and the trigeminal ganglia, and in spinal cord in the dorsal and ventral root, i.e. in the ascending and descending pain system (5).CRPS is a relatively common outcome after surgery and trauma and often treatment-resistant. This case report shows that topical menthol could be highly potent component in multimodal pain management in persistent post-operative/post-traumatic pain.References1. CIPN occurs in 58-78% of patients who receive eg. carboplatin and paclitaxel, and 1/3 of patients have symptoms 6 months after chemotherapy. 1Recent interest on menthol in pain management has risen since 2002 when the menthol sensing channel transient receptor potential melastatin 8 (TRPM8) was identified. 2Here we present a case report where menthol-cream was used in the treatment of refractory CIPN pain.Methods:A 49 years old women underwent debulking surgery for ovarian cancer and peritoneal carcinosis on 7/2017. After surgery, she had chemotherapy with carboplatin and paclitaxel. On 8/2018 she had no sings of recurrence of the cancer, but severe pain in the hands and feet. Ibuprofen, oxycodone-naloxone and venlafaxine provide only 30% pain relief. Her pain on NRS was on average 4/10, least 2/10 and most 6/10. Her pain interfere with several daily functions, mood and normal daily activities most severely affected.She was prescribed menthol-cream (Ice Poweru00ae, Fysioline Ltd., Tampere, Finland) to be used 2-4-times a day.Results:After 4 weeks use of menthol-cream she was able to stop other analgesics. On 12/2018, after 4 months use of menthol-cream, she had neither pain nor any other symptoms of neuropathy. Her life satisfaction (LS) on the 4-item LS-scale (4-20) 3 had improved, score before menthol-cream was 8/20 and after it 6/20.Conclusions:In consistent with other reports 4, in this case topical menthol was highly effective on CIPN.References1. Pain is both a symptom and a disease. It manifests in multiple forms and its treatment is complex. Physical, social, economic, and emotional consequences of pain can impair an individual's overall health, well-being, productivity, and relationships in myriad ways. The impact of pain at a population level is vast and, while estimates differ, the Centers for Disease Control and Prevention reported that 50 million U.S. adults are living in pain. In terms of pain's global impact, estimates suggest the problem affects approximately 1 in 5 adults across the world, with nearly 1 in 10 adults newly diagnosed with chronic pain each year. In recent years, the issues surrounding the complexity of pain management have contributed to increased demand for alternative strategies for treating pain. One such strategy is to expand use of topical pain medicationsâ€"medications applied to intact skin. This nonoral route of administration for pain medication has the potential benefit, in theory, of local activity and fewer systemic side effects. The brain is the most complex organ in our body. Indeed, it is perhaps the most complex structure we have ever encountered in nature. Spasmodic torticollis, also known as cervical dystonia, affects about three people in 10,000, or an estimated 85,000 individuals in the United States alone. Despite this, there has been until now a lack of information outside of the professional medical literature for use by individuals with this disorder and their families. This book provides comprehensive information on the disorder for people with spasmodic torticollis and those close to them. Medical terms and concepts are introduced sequentially and then used as building blocks for the later discussion. Beginning with a clear definition of the disorder, opening chapters categorize this neurologic disease as one of the broader category of movement disorders, and differentiate it from other conditions with which it is often confused. The authors then present a stepwise introduction to the relevant anatomy and physiology of the nervous system and neck. They draw on the experiences of their patients to build a progressive depiction of the experiences an individual might have as he or she goes through the initial onset of symptoms, progression of the disorder, seeking medical care, diagnosis, treatment, and subsequent outcome. Personal vignettes from the experiences of selected patients are provided where they illustrate particular points in the discussion. Subsequent chapters discuss various modes of treatment for spasmodic torticollis. Prior to the mid-1980?s, there were no specific treatments for this disorder. Nearly all treatment consisted of using oral medications that were primarily intended for other medical conditions. Since most of these medications are still in use, and a few new ones have been added, a chapter is devoted to detailing them and discussing the general principles of medication therapy. During the past decade, chemodenervation using botulinum toxin has become the primary and most effective treatment for spasmodic torticollis. For those few patients who require surgery, a description is provided of the neurosurgical techniques developed during the last twenty years specifically for its treatment. The final chapter is a manual of therapeutic rehabilitation exercises designed to alleviate the symptoms of spasmodic torticollis. These exercises can be performed by most patients with no assistance and a bare minimum of equipment. Since each person?s case of spasmodic torticollis is different, only certain of the exercises may be appropriate for any given individual. They should be undertaken only after discussion with your physician. These exercises are accompanied by detailed illustrations that emphasize the particular muscles relevant to each posture or movement. About the Authors: Dr. Pathak is a neurologist with a special interest in the neurologic rehabilitation of movement disorders, especially spasmodic torticollis. Dr. Frei is a neurologist specialized in the field of neurogenetics, and has conducted clinical trials on a number of movement disorders, including spasmodic torticollis. Dr. Truong is a neurologist and movement disorders specialist. He has conducted active research in the management of movement disorders, including spasmodic torticollus. Pain is a symptom of many clinical disorders, afflicts a large proportion of the population and is largely treated by pharmacological means. However, the two main classes of drugs used are the opioids and the non-steroidal anti inflammatory drugs, drugs that have a long history. The last decade has seen remarkable advances in our understanding of some of the pharmacological bases of pain and analgesia and this book aims to reflect these rapid changes in our understanding of pain mechanisms. One impetus to these scientific advances has been dialogue and interactions between scientists and clinicians; as a result we now has a number of animal models of clinical pain states, to mimic certain aspects of clinical pathophysiological pain states. Molecular aspects of receptors and the synthesis of tools for probing receptor function have also been rapid growth areas. A number of controlled clinical studies using novel licensed drugs have also resulted from recent research, offering hope to certain patients with severe intractable pain. However, we desperately need the pharmaceutical industry to develop new drugs based on these novel targets for analgesic therapy. This book attempts to provide an overview of the important areas of the pharmacology of pain. This book, although providing an account of the pharmacology of pain transmission and its control based on the underlying anatomical organization and physiological responses, does not attempt to cover these latter two areas.
