Methadone-associated overdose deaths (MAOD) -- those in which methadone may have caused or contributed to the death -- have risen sharply. Before the late 1990s, methadone was used mainly to treat opioid addiction but has since been increasingly prescribed to manage pain. Taken too often, in too high a dose, or with other drugs or alcohol, methadone can cause serious side effects and death. MAOD can occur under several different scenarios, incl. improper dosing levels by practitioners, misuse by patients who may combine methadone with other drugs, or abuse -- using the drug for non-therapeutic purposes. This report examines the reg¿n. of methadone, factors that have contributed to the increase in MAOD, and steps taken to prevent MAOD. Ill.

Methadone is a long-acting opioid medication that is used as a pain reliever and, together with counselling and other psychosocial services, is used to treat individuals addicted to heroin and certain prescription drugs. Methadone is regulated as a controlled substance, under federal and state laws and regulations, when used for pain management and addiction treatment. Although information on methadone-associated overdose is limited, available data suggest that methadone's growing use for pain management has made more of the drug available, thus contributing to the rise in methadone-associated overdose deaths. This book examines the regulation of methadone, factors that have contributed to the increase in methadone-associated overdose deaths, and steps taken to prevent such deaths from occurring. The opioid crisis in the United States has come about because of excessive use of these drugs for both legal and illicit purposes and unprecedented levels of consequent opioid use disorder (OUD). More than 2 million people in the United States are estimated to have OUD, which is caused by prolonged use of prescription opioids, heroin, or other illicit opioids. OUD is a life-threatening condition associated with a 20-fold greater risk of early death due to overdose, infectious diseases, trauma, and suicide. Mortality related to OUD continues to escalate as this public health crisis gathers momentum across the country, with opioid overdoses killing more than 47,000 people in 2017 in the United States. Efforts to date have made no real headway in stemming this crisis, in large part because tools that already existâ€"like evidence-based medicationsâ€"are not being deployed to maximum impact. To support the dissemination of accurate patient-focused information about treatments for addiction, and to help provide scientific solutions to the current opioid crisis, this report studies the evidence base on medication assisted treatment (MAT) for OUD. It examines available evidence on the range of parameters and circumstances in which MAT can be effectively delivered and identifies additional research needed.

With widespread clinical utilization of methadone as a treatment modality for heroin addiction, public attention has been drawn increasingly of late to its potential for diversion to nontherapeutic situations. Deaths directly related to the abuse of black-market methadone have received considerable notoriety in the District of Columbia and elsewhere. Much can and has been learned relating to the complex epidemiological dynamics of the situation, where careful examination of a cohort of methadone-related fatalities has afforded promulgation of reasoned public policy decisions relative to community methadone utilization. Countermeasures taken to control the problem here have proven remarkably effective.
