Military Radiobiology provides an understanding of the sources and consequences of radiation exposure. Military personnel must develop a working knowledge of postexposure effects in order to determine points of intervention. The medical problems confronting military radiobiology include target damage, which causes decrements in normal performance, physiological injury, and impairments of the immunological-hematological system that lead to life-threatening infectious complications. The book begins by describing the properties of nuclear weapons, including the mechanisms by which nuclear energy is stored within the nucleus, its release, and its conversion to those forces associated with nuclear weapons. The most important consideration in the medical evaluation of a radiation event is the relative magnitude of the situation and the resources needed to address the emergency. In many cases, order of magnitude estimates of the scale of the event will be sufficient for emergency response. Small-scale events are those occurring in laboratories, hospitals, nuclear power plants, etc., involving small amounts of radionuclides with the potential exposure and/or contamination of one or a few individuals. Large-scale events are those involving relatively large quantities of radionuclides and the potential exposure or contamination of large numbers of people, e.g., terrorist attacks with radiological weapons, nuclear weapons detonation, and large-scale nuclear power plant disasters. High-level external ionizing radiation poses the greatest danger to living organisms. Low levels of internal or external contamination generally pose very low risk. A site known to be radiologically contaminated should be surveyed before entering and responders should be advised to limit their time in high dose-rate areas. There is generally no hazard associated with handling a radiologically contaminated casualty. U.S. military planning. The U.S. military has established a system for mission-specific risk-based dose limits that includes life-saving activities. In current doctrine, U.S. military personnel become restricted from engaging in operational radiological/nuclear missions once they have exceeded a 125-cGy dose accumulation. Miniature pigs were trained by shock avoidance conditioning to traverse, on cue, a two-chambered shuttlebox and performance was evaluated after the pigs received mid-line tissue doses of pulsed, mixed gamma-neutron radiations ranging from 1000 to 14,700 rads. At 2400 rads or more, most pigs were incapacitated with severe convulsions almost immediately after irradiation. At doses of 2400 to 5000 rads this incapacitation often lasted less than 5 minutes after which the animals began to recover and perform. At higher doses, most animals were more severely affected. They became comatose and did not begin to recover until 15 to 50 minutes postirradiation. After recovering from early transient incapacitation the pigs worked for a time at relatively normal levels. Later their performance degenerated and the pigs became permanently incapacitated a few hours before death. At doses of 7600 rads and higher, some of the animals recovered only partially from the early transient incapacitation before becoming permanently incapacitated; and at 13,200 rads or more, most animals were permanently incapacitated immediately after irradiation. In support of its mission the Armed Forces Radiobiology Research Institute (AFRRI) operates a medium sized research nuclear reactor. The reactor is used to generate radiations, primarily neutrons and gamma rays, which are used to conduct experimental biomedical research and to produce isotopes. The radiations are delivered to the experiments in one of two ways: A pulse operation delivers a very short burst of high power, or a steady state operation delivers a longer, continuous low- to medium-power exposure. The reactor is also used to train military personnel in reactor operations. TRIGA is an acronym for Training, Research, and Isotope, General Atomics. Mark-F is the specific General Atomics Reactor model, distinguished by a pool, a movable core, exposure room facilities, and the ability to pulse to momentary high powers. Reactor operations at AFRRI began is 1962. In 1965, a change was made from aluminum clad to stainless steel-clad fuel elements.
