Healthcare support workers (and assistant practitioners) provide a vital service to help ensure person-centred care. They are a central part of a diverse workforce that is becoming more complex as the role of nurses continues to develop and new environments of care emerge. We now have a comprehensive UK wide evidence base to inform the discussions with our key stakeholders which will allow us to consider how we take this important work forward. Any future model of regulation for healthcare support workers must be proportionate and flexible enough to work across all of the UK's healthcare settings without compromising patient safety. The public need to have confidence that the people who care for them are competent and that there are robust systems in place to address issues of poor conduct or competence. We will therefore be taking a risk-based approach in order to ensure that the public are appropriately safeguarded.

Context is a central concept in organization and management studies, yet it is often used in a generalized, unspecific manner. The Modernization of the Nursing Workforce: Valuing the healthcare assistant is based on recently completed research exploring the role of healthcare assistants (HCA) in acute hospitals. Whilst a support role working alongside registered nurses has been a longstanding feature of the NHS, the contemporary HCA role has become increasingly central to the process of health service modernization. The role is now assuming even greater importance as the ramifications of financial constraints, restructuring and other pressures on the NHS play out. The issue is becoming increasingly relevant as the government has commissioned an independent review into the role of healthcare assistants, the Cavendish Review, which uses this book extensively. The HCA role is unregulated and low paid, but by taking-on direct care tasks from registered nurses, the role has become politically sensitive. The HCA remains a cheap and flexible source of labour, but the unregulated role encourages dilemmas and public scrutiny over risk and patient safety. The book explores how public policy reform of the health service feeds through to impact upon the management and structure of the healthcare workforce. More specifically, the book provides a timely evidence base for the extended and growing use of the HCA role. The book draws upon a multi-method research design from four geographically located hospital trusts in England, which during a three year period saw over 270 staff interviewed, focus groups and interviews with over 100 patients, some 275 hours of ward-based observation, and detailed survey responses from over 3,000 members of staff and hospital patients. The unusual richness of the data allows a definitive examination of who undertakes the HCA role, its shape, nature and diversity, along with the consequences for those with a stake in the role - hospital managers, the assistants themselves, the patients they care for and the nurses they work alongside, making The Modernization of the Nursing Workforce: Valuing the healthcare assistant essential reading for health care studies and public management communities, and those charged with training and education policy.

The Health Committee reports on the annual accountability hearings with the General Medical Council (GMC) (HC 1429) and Nursing and Midwifery Council (NMC) (HC 1428). The reports call for doctors and nurses to develop a wider responsibility for the overall quality of care delivered to patients and they have an obligation as professionals to report to their professional body any concerns they have about the quality of care being delivered by their colleagues. The GMC and the NMC must give a strong lead in this area and ensure that failure to act on this responsibility is regarded as a serious breach of professional obligation. The Committee stresses that both regulators need to have effective revalidation processes in place so that they can periodically check on how doctors and nurses are performing. The Committee recognises that the NMC is making steady progress towards being an effective regulator but cautions that there remains substantial ground to cover before it can be considered fully effective: work around pro-active regulation (risk-based visits) must be expanded; guidance about the care of older people must be reinforced by an action plan to deliver improved outcomes in this group. Government and both regulators must speed up efforts to resolve the serious problems posed by doctors and nurses who qualify elsewhere in Europe, and earn the right to work in the UK without having their language or medical skills tested. The UK and European law that underpins the workings of both regulators needs a complete overhaul.
