Myanmar's National Health Plan (NHP) for 2017-2021 has laid out the vision of achieving Universal Health Coverage (UHC) by 2030. The NHP aims to improve the delivery of health services and financial protection for Myanmar people through substantial investments in frontline service delivery units and through a range of reforms in the health system, including on health financing. This report assesses Myanmar's health financing system. The analysis is structured around three main sets of questions: (i) Who pays for health in Myanmar? Given that the government needs to invest more in the health sector, where could (or should) the money come from; (ii) Are prepaid and pooled funds for health sufficient and equitable? What additional pooling arrangements could Myanmar consider; and (iii) What key steps and reforms are needed for Myanmar to develop the capabilities of a strategic purchaser in the medium term? This Health Financing System Assessment aims to inform health financing policy choices that the Government of Myanmar will need to make as part of the development and implementation of its Health Financing Strategy.

The health sector of Papua New Guinea (PNG) has seen significant positive developments in recent years. Key indicators of health access and quality have, however, barely improved or have even declined (for example, maternal and child health) from 2006 to 2015. In 2016, PNG entered the accelerated transition phase from the vaccine alliance (Gavi) support. During this time, the government will be expected to increase its share of co-financing for vaccines, while Gavi gradually decreases their contribution. The government is simultaneously moving towards universal health coverage (UHC), along with many other developing countries, and has recently established fee free primary health care and subsidized secondary care. Given this backdrop, the health financing system assessment (HFSA), which evaluates the financing system and institutional sustainability, comes at an opportune time. The report begins by providing a comprehensive background, including an overview of PNG's economic situation, health demographics, health financing, human resources for health and the health system. It then analyses two areas critical to sustainable health financing: (i) PNG expenditure, with a focus on levels and sources of health expenditure, as well as resource allocation; and (ii) PNG reliance on donor resources, in particular, the global fund to fight aids, tuberculosis, and malaria (GFATM) and Gavi.

It has been more than 20 years since Brazil's 1988 Constitution formally established the Unified Health System (Sistema Unico de Saude, SUS). Building on reforms that started in the 1980s, the SUS represented a significant break with the past, establishing health care as a fundamental right and duty of the state and initiating a process of fundamentally transforming Brazil's health system to achieve this goal. This report aims to answer two main questions. First is have the SUS reforms transformed the health system as envisaged 20 years ago? Second, have the reforms led to improvements with regard to access to services, financial protection, and health outcomes? In addressing these questions, the report revisits ground covered in previous assessments, but also brings to bear additional or more recent data and places Brazil's health system in an international context. The report shows that the health system reforms can be credited with significant achievements. The report points to some promising directions for health system reforms that will allow Brazil to continue building on the achievements made to date. Although it is possible to reach some broad conclusions, there are many gaps and caveats in the story. A secondary aim of the report is to consider how some of these gaps can be filled through improved monitoring of health system performance and future research. The introduction presents a short review of the history of the SUS, describes the core principles that underpinned the reform, and offers a brief description of the evaluation framework used in the report. Chapter two presents findings on the extent to which the SUS reforms have transformed the health system, focusing on delivery, financing, and governance. The global movement toward universal health coverage (UHC) is accompanied by requests for large increases in government health spending in some countries. This combined with the global economic situation and stagnant economic growth across many low- and middle-income countries make it more critical than ever to place health financing discussions firmly in the context of macroeconomic and fiscal realities. Unfortunately, there is often a disconnect in decision making, with key fiscal decisions made in the absence of a clear understanding on the one hand of the potential consequences for the health sector, and on the other, the consequences for the country’s macroeconomic and fiscal position of increasing or reallocating government spending. Constructive health financing policy dialogue aims to reach a common understanding between health sector leaders and central budget authorities about policy objectives for the health sector and the resources needed to achieve those objectives, how much priority will be given to health in the government budget, and how the health sector will be held accountable for using funds effectively. This common understanding should be built on a realistic picture of the country’s macroeconomic and fiscal context, the constraints and competing priorities in the budget-setting process. When ministries of health and ministries of finance have a common understanding of macroeconomic and fiscal constraints, discussions can focus productively on using funds within the potential health resource envelope in the most effective way to achieve health system objectives. This guidance note outlines the key components of the macroeconomic, fiscal, and public financial management context that need to be considered for an informed health financing discussion at the country level. The guidance note is organized around four sets of questions that are key to placing the health financing dialogue in the context of a country’s macroeconomic and fiscal context. Each section points to measures, resources, and analytical tools that are available to assist in answering these questions for a specific country. The guidance note draws on case studies from 11 countries moving toward or sustaining universal health coverage conducted as part of the Japan†“World Bank Partnership Program on UHC as well as from other country examples.
