Ischemie heart disease is still the most frequent cause of death in the western world. There have been significant achievements in diagnostic procedures as well as in the medical, invasive, and surgiealtreatment ofischemieheart disease inrecent years. A variety ofdrugs are availablefor the pharmacotherapyofischemieheart disease, par ticularly nitrates, ß-blockers, and calcium-antagonists which are used as mono therapy or in various combinations. However, the selection of patients for a certain treatment, as weIl as the optimization of an individual treatment are still largely empirical. On the other hand, the recent advances in experimental cardiology emphasize the extremely complex and dynamie scenario of ischemic heart disease, involving endothelial damage, coagulation processes, metabolie and morphologie derangements, coronary constrictor mechanisms, blood flow redistribution, arrhythmogenesis, contraetile dysfunction during ischemia and reperfusion, and finally lack or presence of pain perception. Therefore, it appears desirable to close the gap between experimental and clinical cardiology and, thus, to provide a pathophysiologieal basis for rational cliniealdecisions with respect to diagnostic and therapeutic procedures. The idea for this book arose during the preparation of a seminar series on experi mental cardiology, when I found it diffieultto collect the pertinentinformation from textbooks of cardiology, physiology, pathology, and pharmacology, as well as from numerous review and original artieies on specifictopies. Current evidence suggests that the ischemic preconditioning response is a multi-factorial process consisting of an initial early trigger, an intermediate mediator, and an end effector. Each of these steps in is now its own intense area of investigation. The need to render the heart ischemic for a brief period to invoke the preconditioning response is currently the major factor limiting clinical application of this powerful cardioprotective strategy. Recent research efforts have utilized brief exposures to pharmacological agents, in lieu of a brief preconditioning ischemia, to trigger/mimic the ischemic preconditioning-induced response. The World Heart Congress held in Winnipeg in July 2001 provided a forum for the presentation of new insights into the basic mechanisms of ischemia and reperfusion injury, as well as novel strategies to protect the heart from cell death, ventricular arrhythmias, and contractile dysfunction. Many pioneers in the fields of ischemia-reperfusion injury and preconditioning-induced protection presented there and the chapters in this book represent selected papers from these symposia.

The articles collected in this volume largely arose from two related meetings held last spring. The first was held in Buenos Aires, Argentina on April 11-12, 1997 and was titled Nuevos Avances en el Fenómeno de Isquemia y Reperfusión (New Advances in the Phenomenon of Ischemia and Reperfusion). The second meeting took place in Stará Lesná located in the High Tatras Mountains of the Slovak Republic on June 27-30, 1997. The coronary circulation is central to sustaining myocardial viability. Unlike the cir culations of most other organs, if the coronary circulation becomes insufficient to sustain myocardial contractile function, overall quality of life rapidly declines and life can abruptly end. Over the past years, the thrust of research in cardiology has been toward an understanding of the engineering of the heart as a pump that transports blood to the various organs of the body. More recently, the fields of biochemistry and biophysics have come to influence heart research. The modern cardiologist can no longer pretend to understand, for example, what is happening to the patient with myocardial infarction or ischemia without understanding the principles of molecular biology. The structure and function of the heart are therefore central themes of cardiological research and practice, which incorporate knowledge and discoveries from diverse disciplines. The importance of lipid metabolism in the myocardium has become clearly understood. In the well-oxygenated heart, fatty acids are the preferred substrates. The fact that the heart derives most of its energy from the oxidation of lipids, which represent the larg est energy store of the body, is logical for an organ that must work throughout our lifetime. There are, however, several lines of evidence that during ischemia, lipids may. be harmful to the heart. High levels of free fatty acids in the serum have been suggested to play a role in causing life-threatening arryhthmias and damage in the ischemic heart. The molecular basis for these effects remains poorly understood, and several possible mechanisms for these harm ful effects have been suggested.

Cardiac Ischemia: From Injury to Protection has been divided into six parts. The first part describes the differences between hypoxia and ischemia, animal models, the effects of ischemia on myocardial function and metabolism, and the electrophysiological consequences of ischemia. The second part deals with the mechanisms of cardiomyocyte death in ischemia, structural aspects of irreversible ischemic injury, necrosis and apoptosis of cardiac cells, the role of calcium, and the concept of calcium antagonism. The third chapter is a brief description of reperfusion injury, its clinical relevance, and possible prevention. The fourth part summarizes changes in myocardial vasculature during ischemia and reperfusion. The fifth part is the survey of two main possibilities for increasing cardiac resistance to ischemia and hypoxia, i.e. long-lasting adaptation to chronic hypoxia and short-lasting preconditioning. The last part of the book deals with comparative and ontogenetic aspects of cardiac sensitivity to oxygen deprivation; this chapter also summarizes the ontogenetic differences and limitations in endogenous and exogenous protection of the ischemic/hypoxic heart.
