Este livro é fruto da pesquisa de doutorado da autora e aborda a complexidade dos fatores psíquicos implicados na vivência das mães de bebês prematuros em relação à amamentação, em uma Unidade Neonatal do Distrito Federal. A amamentação vai além de um processo meramente fisiológico, exigindo da mulher condições psíquicas favoráveis para que ela possa desempenhar o papel de nutriz. No caso de nascimentos prematuros, podemos questionar se tais condições são esperadas no contexto de internação do recém-nascido. Neste trabalho foram apresentados três estudos de caso. No primeiro, enfatizamos o quanto a relação da mãe com a equipe de saúde pode despertar sentimentos intensos e de difícil contenção. No segundo, ressaltamos a importância de o bebê ser considerado um parceiro ativo na relação mãe-filho, podendo influenciar no processo da amamentação e, no terceiro, abordamos a importância de fatores subjetivos e inconscientes que interferem no desejo da mulher de amamentar a criança. Durante o período em que a pesquisa foi desenvolvida, apesar de não ter sido evidenciado um discurso impositivo dos profissionais em relação à amamentação, concluímos que manifestar o não desejo em aderir a essa prática, principalmente quando se trata de mães de bebês prematuros, pode adquirir um significado socialmente não aceito e contribuir para a intensificação do sentimento de culpa existente nessas mulheres, que foram feridas em seu narcisismo.

In this open access book the NIHR Global Health research group on preterm birth, PRIME, prioritizes the development of prenatal risk assessment guidance for preterm birth. The increasing prevalence of preterm birth in the United States is a complex public health problem that requires multifaceted solutions. Preterm birth is a cluster of problems with a set of overlapping factors of influence. Its causes may include individual-level behavioral and psychosocial factors, sociodemographic and neighborhood characteristics, environmental exposure, medical conditions, infertility treatments, and biological factors. Many of these factors co-occur, particularly in those who are socioeconomically disadvantaged or who are members of racial and ethnic minority groups. While advances in perinatal and neonatal care have improved survival for preterm infants, those infants who do survive have a greater risk than infants born at term for developmental disabilities, health problems, and poor growth. The birth of a preterm infant can also bring considerable emotional and economic costs to families and have implications for public-sector services, such as health insurance, educational, and other social support systems.
