How resources are being used to fund the community response to human immunodeficiency virus (HIV) and acquired immune deficiency syndrome (AIDS) is of considerable interest to the donor community and governments. In the past decade, international funding for the HIV and AIDS response provided by governments rose from about US$1 billion to US$8.7 billion; donors increasingly shifted their financial support toward funding community responses to this epidemic. Yet little is known about the global magnitude of these resource flows and how funding is allocated among HIV and AIDS activities and services. Although some studies have been carried out to gather information on the community response by civil society organizations (CSOs), most of them provide only partial information limited to a specific intervention (for example, orphan support) or specific local communities. To address this knowledge gap, the report attempts to answer the following questions: How large is donor funding for community-based interventions that are run by either large nongovernmental organizations (NGOs) or smaller community-based organizations (CBOs)? How do the funds reach various types of CSOs? What are CSOs' other sources of funding, and to what extent are the CSOs dependent on donor funding? How are these funds used for by CSOs? Are there differences among different types of CSOs working on HIV and AIDS?

This report presents the findings from a six-country study of access to AIDS funding by civil society organisations in southern Africa. Focusing on the period 2001-2005, when funding for AIDS increased dramatically in the region, it examines the AIDS funding environment through a civil society lens, drawing upon data collected from more than 400 organisations in the region, as well as interviews with donor institutions and community-level case study research. It depicts a complex picture of a changing resource environment, characterised by efforts to channel funding closer to the ground, but continuing difficulties on the part of small organisations to access resources in forms and amounts suitable for their institutional needs.

In the past decade the global financial assistance for AIDS responses increased tremendously and the donor community provided greater resources to community responses. Yet little is known about the global magnitude of these resources and their allocation among HIV and AIDS activities and services. To address this knowledge gap, this report pulls together evidence from several different sources (donor data bases, surveys of civil society organizations, country funding profiles) to determine, among other things, how funds are reaching civil society and community-based organizations, how these funds are being used, and the degree to which these organizations rely on other sources of funding. The analysis suggests that funding flows have increased dramatically for civil society organizations (CSOs), reaching at least $690 million per year on average during the period 2003-2009. However, much smaller funding is reaching organizations at local level.The report documents the impact achieved by this funding. The report argues that current evidence shows that community responses play a useful complementary role to national AIDS programs that has been achieved with relatively little funding. Contrary to a widespread view, the report highlights that community responses add resources to national programs. In Kenya, Nigeria and Zimbabwe, substantial mobilization of resources in the form of volunteers are mobilized by communities. There is a strong risk that in the current environment of increased resource scarcity, prevention programs implemented by civil society organizations would be cut unless there is strong evidence of value for money being generated. Community-based organizations are ill equipped to answer that question, but there is scope for improving the results that they generate. The report argues that improving coordination with national programs, strengthening consistency between local activities and HIV epidemics, building stronger network of civil society organizations, and mobilizing sustainable funding are the most important ways for community responses to move forward and address the challenges faced by community responses.

Civil Society Organizations (CSOs) can make a vital contribution to public health and health systems but harnessing their potential is complex in a Europe where government-CSO relations vary so profoundly. This study is intended to outline some of the challenges and assist policy-makers in furthering their understanding of the part CSOs can play in tandem and alongside government. To this end it analyses existing evidence and draws on a set of seven thematic chapters and six mini case studies. They examine experiences from Austria Bosnia-Herzegovina Belgium Cyprus Finland Germany Malta the Netherlands Poland the Russian Federation Slovenia Turkey and the European Union and make use of a single assessment framework to understand the diverse contexts in which CSOs operate. The evidence shows that CSOs are ubiquitous varied and beneficial and the topics covered in this study reflect such diversity of aims and means: anti-tobacco advocacy food banks refugee health HIV/AIDS prevention and cure and social partnership. CSOs make a substantial contribution to public health and health systems with regards to policy development service delivery and governance. This includes evidence provision advocacy mobilization consensus building provision of medical services and of services related to the social determinants of health standard setting self-regulation and fostering social partnership. However in order to engage successfully with CSOs governments do need to make use of adequate tools and create contexts conducive to collaboration. To guide policy-makers working with CSOs through such complications and help avoid some potential pitfalls the book outlines a practical framework for such collaboration. This suggests identifying key CSOs in a given area; clarifying why there should be engagement with civil society; being realistic as to what CSOs can or will achieve; and an understanding of how CSOs can be helped to deliver.

The Department for International Development (DFID) launched its new HIV/AIDS Strategy "Achieving Universal Access: the UK's strategy for halting and reversing the spread of HIV in the developing world" in June 2008. DFID is widely acknowledged as a global leader in tackling HIV/AIDS, particularly amongst vulnerable and marginalised groups, including women and children. Its Strategy provides an excellent analysis of the challenges faced in tackling HIV/AIDS effectively. It makes substantial financial commitments, most notably £6 billion over seven years to strengthen health systems in partner countries, and £1 billion over the same period for the Global Fund to Fight AIDS, Tuberculosis and Malaria. Direct and specific HIV/AIDS funding of this kind continues to be necessary to fill the gaps in prevention and treatment services in high-prevalence countries. But the Strategy is strong on rhetoric but weak in communicating how DFID will implement it. There are few measurable targets or indicators of how the Strategy's effectiveness will be assessed. DFID fails to explain how the high-level funding commitments will be broken down by country or sector, making it difficult to understand how implementation will occur on the ground. The Committee has concerns that social protection programmes, which are now DFID's main instrument for assisting children orphaned and made vulnerable by HIV/AIDS, will not be specifically targeted at this vulnerable. The overall aim of the Strategy is universal access to HIV prevention, treatment and care, but the target date for achieving this is only two years away in 2010.
