Although just two years have passed since the first English edition of this book, advances in neurophysiology have dictated considerable revision of most of the chapters. The chapters on synaptic transmis sion, motor systems, and the autonomie nervous system, for example, have been revised, extended, and in some parts entirely rewritten. In response to a frequently expressed wish, a chapter on the in tegrative functions of the nervous system has been added. Here the use of the term "integrative functions" expresses our lack of a better general term covering such diverse activities and states of the nervous system as waking, sleeping, dreaming, consciousness, speech, leam ing, and memory. This chapter also includes an introduction to the physiology of the cerebral cortex and the characteristics of the elec troencephalogram. Another new section is a chapter on the control-systems aspects of central nervous activity, a reßection of the fact that many processes, particularly those involving motor activity and the autonomie nervous system, can best be described and analyzed in terms of control theory. The previous Chapter 7, Sensory Systems, has been largely included in another volume, "Fundamentals of Sensory Physiology." Finally-again at the suggestion of readers-a bibliography has been added to guide the student further into the topics of the indi vidual chapters. Most of the references are re cent; they offer access to the current originalliterature.

Since 1978 this textbook, to the gratification of its authors and publisher, has found an undiminished readership. Recent research in sensory physiology has progressed so rapidly that this third edition, like the second, has required thorough revision. The understanding of pain, in particular, has increased to a remarkable degree. This development is reflected here in the appearance, for the first time, of a chapter devoted entirely to the subject "Nociception and Pain". In view of the great clinical significance of pain, it seemed necessary to broaden the scope of the discussion, so that in addition to the aspects directly related to sensory physiology consideration is given to the pathophysiology, pharmacology and psychology of pain. The chapters present in earlier editions have also been carefully reexamined and, where necessary, revised and extended. Most of the illustrations provided for the first edition by the Stuttgart studio Gay & Benz have been retained. Some required alteration or replacement, and a number of new illustrations have been added. For the meticulous skill with which she transformed our ideas into graphs and drawings, we are most grateful to Mrs. Regine Gattung-Petith.

Again rapid advances in the brain sciences have made it necessary, after only a few years, to issue a revised edition of this text. All the chapters have been reviewed and brought up to date, and some have been largely rewritten. The major revision has occurred in the chap ters on the autonomic nervous system and the integrative functions of the central nervous system. But in the discussion of the motor systems and other subjects as well, recent insights have necessitated certain conceptual modifications. In the description of the autonomic nervous system, the role of the intestinal innervation has been brought out more clearly than before. In addition, there is a new presentation of the physiology of smooth muscle fibers, and more attention has been paid to the postsynaptic adrenergic receptors, because of the increasing therapeutic signifi cance of the at f3 receptor concept. A substantial section on the genital reflexes in man and woman, including the extragenital changes during copulation, has also been added. The text on the integrative functions of the central nervous system has been expanded to include, for the first time, material on brain metabolism and blood flow and their dependence on the activity of the brain. Reference is also made to recent results of research on split brain and aphasic patients and on memory, as well as on the physiol ogy of sleeping and dreaming.

Clinical neurophysiologic testing plays a critical role as a complement to the clinical assessment in patients who are being evaluated for a variety of neurologic symptoms. Many different techniques and methods of assessment can be used to evaluate the function of the nervous system, including electroencephalography, electromyography, evoked potentials, movement disorder studies, and sleep studies. An accurate understanding of the role of these tests and reliable technical performance and interpretation of these studies is critical in clinical practice. This new edition in the Contemporary Neurology Series remains an essential resource for physicians and technologists learning or utilizing clinical neurophysiology in their training or practice. This fifth edition updates the basic concepts underlying each of the techniques used in clinical neurophysiology and provides detailed descriptions of the methods, findings, studies, and value of the wide range of electrophysiologic testing available for patients with epilepsy and spells, neuromuscular diseases, movement disorders, demyelinating diseases, sleep disorders, autonomic disorders, and those undergoing orthopaedic and neurosurgical procedures in the operative setting. The role of each type of study, the interpretation of findings, and the application of the studies to different types of clinical problems are detailed throughout the text. It is a practical textbook for neurologists, physiatrists and clinical neurophysiologists in clinical or research practice or in training.
