Excerpt from New Yorker Medizinische Monatsschrift, Vol. 16: Januar-Dezeber 1904 Die eigentumliche Verkennung der hier in Betracht kommen den Verhaltnisse wurde zum Teil herbeigefuhrt durch die Ver wirrung, die sich an die Diskussion uber den klinischen Begriff der Magenerweiterung anschloss. Ich will auf dieses Kapitel nicht weiter eingehen, als zur Deutung des Magensaftfiusses not wendig ist. Es besteht keine Veranlassung, wie dies neuerdings oft vorgeschlagen wurde, den Begriff "magenerweiterung ganz abzuschaffen. Das Bild der Magenerweiterung ist klinisch scharf umgrenzt und ist charakterisiert durch die Kombination zweier Zustande, welche man an sich wohl auseinanderhalten muss, nam lich einmal, die Erweiterung des Organes, und zweitens die Stagnation von Mageninhalt. Die Erweiterung des Magens ist ein anatomischer Zustand. Die Stagnation des Mageninhalts stellt eine funktionelle Storung dar. Ein Magen mit normaler motorischer Tatigkeit, gleichgultig wie gross er ist, entleert sei nen Inhalt uber Nacht; wo das nicht geschieht. Besteht eine motorische Insufficienz. Die stagnierenden Massen konnen haupt sachlich bestehen aus soliden Nahrungsresten gemischt mit mehr oder weniger grossen Mengen Flussigkeit, oder hauptsachlich aus Flussigkeit, der wenig oder gar keine soliden Teile beige mischt sind. Wo Stoffe gefunden werden, besteht bezuglich des Begriffes der Stagnation keine Meinungsverschiedenheit. Wo aber nur Flussigkeit aus dem nuchteri1en Magen ausgehebert wird, wie bei vielen Fallen von Magensaftfiuss, da glaubt man den Be griff der Stagnation ablehnen zu durfen, weil der Flussigkeit keine soliden Stoffe beigemengt sind. Juni 1894 durch Symphysiotomie entband. Die Krankenge schichte und die Operation wurde im Medical Record vom 10. November beschrieben. Es war mir damals rathselhaft warum Pat., die sich rasch erholte und eine gute Verbindung der Symphyse hatte, und bei der auch keine Aifection der Iliosocralgelehke nachzuweisen war, einen bedeutend wat schelnden Gang hatte und fortfuhr, Schmerzen im Becken zu haben. Sie wurde am 1. August entlassen. Anfang December stellte sie sich mir wieder vor und hatte dann eine entzundliche Beckengeschwulst von der Grosse von zwei Fausten und be hauptete, dass wiederholt Eiter aus der Scheide abgegangen sei. Aus dem Nachfolgenden wird aber hervorgehen, dass es eine seriose Flussigkeit gewesen sein muss. Dann trat die Menstruation ein, und wahrend sie lag, fuhlte sie eine Nacht wieder, dass etwas durch die Scheide abging, awas ihr grosse Erleichterung verschaffte, und wonach die Geschwulst sich auf weniger als die Halfte verkleinert-e.
