The purpose of this book is to report on my 17 years of experience in the development and implementation of a closed functional method of treatment of certain fractures of long bones of the appendicular skeleton. My personal experiences, clinical results, and the basic concepts underlying the treatment philosophy are reported. Illustrations demonstrating the techniques of application of functional casts and braces are presented in detail as well as a step-by-step description of the management regime and a discussion of the behavior of fractures under this system. Indications and contraindications for the use of this method of treatment are clearly outlined. I consider it important to compile this information for I believe that the system has withstood the test of time and is sufficiently sound to be incorporated in the armamentarium of the orthopaedic surgeon. Also, it may have a favorable socioeconomic impact since it offers a viable alternative in the treatment of frac tures.

Unfallchirurgische Therapie steht immer im Spannungsfeld zwischen dem pa thophysiologisch wichtigen Anspruch der frühfunktionellen Behandlung und der Notwendigkeit einer zumindest vorübergehenden Ruhigstellung. Die mo derne Stützverbandtechnik sucht die Ruhigstellung auf das absolut Notwendi ge zu reduzieren und soviel Funktion wie eben möglich zu erlauben. Hierzu gehört auch der vom Patienten oder Physiotherapeuten für gezielte Bewe gungsübungen abnehmbare Stützverband. Die Ruhigstellung angrenzender Gelenke wird ebenfalls auf ein Minimum reduziert. Moderne weiche Stützver bandmaterialien erlauben weiter reduzierte Verbandsformen. So wird die Stützverbandtechnik heute immer mehr dem individuellen Patienten angepaßt und erfordert für die Herstellung ein immer höheres Spezialwissen. Galt bisher der herkömmliche Gips noch als der "goldene Standard", so war innerhalb der letzten 5 Jahre zu beobachten, wie der Gips mehr und mehr durch moderne Kunststoffmaterialien abgelöst wurde. Diese Kunststoff materialien sind nicht nur Gipsersatz, sondern erlauben auch die individuelle Herstellung von abnehmbaren Orthesen, die früher ausschließlich vom Ortho pädietechniker hergestellt werden konnten. So können diese Werkstoffe trotz ihres höheren Preises zur Kostendämpfung beitragen.
