The nineteenth century seems to have been full of hysterical women - or so they were diagnosed. Where are they now? The very disease no longer exists. In this fascinating account, Andrew Scull tells the story of Hysteria - an illness that disappeared not through medical endeavour, but through growing understanding and cultural change. More generally, it raises the question of how diseases are framed, and how conceptions of a disease change through history. The lurid history of hysteria makes fascinating reading. Charcot's clinics showed off flamboyantly 'hysterical' patients taking on sexualized poses, and among the visiting professionals was one Sigmund Freud. Scull discusses the origins of the idea of hysteria, the development of a neurological approach by John Sydenham and others, hysteria as a fashionable condition, and its growth from the 17th century. Some regarded it as a peculiarly English malady, 'the natural concomitant of England's greater civilization and refinement'. Women were the majority of patients, and the illness became associated with female biology, resulting in some gruesome 'treatments'. Charcot and Freud were key practitioners defining the nature of the illness. But curiously, the illness seemed to swap gender during the First World War when male hysterics frequently suffering from shell shock were also subjected to brutal 'treatments'. Subsequently, the 'disease' declined and eventually disappeared, at least in professional circles, though attenuated elements remain, reclassified for instance as post-traumatic stress disorder.

Hysteria is a disease already forgotten by medicine, which, in spite of this, is still very much in vogue. Its name in various academic circles and, especially, in psychoanalytic circles. However, what is today referred to as hysteria is not hysteria, and what is hysteria does not have that name. This book aims to show the form that hysteria actually took before its disappearance in the twentieth century. It aims to answer a simple question. It aims to answer a simple question: What did what was called hysteria for so many centuries look like? What characteristics did it have that identified it from other ailments? How was it dealt with? What was the cause of it? To answer these and other questions, this book makes a historical journey from the first ideas about hysteria, from the first centuries of medicine to the latest conception of it settled in the famous manual of mental disorders, the DSM. This journey is made with emphasis on the second half of the 19th century, the golden age for hysteria and the intellectual environment from which Sigmund Freud and, therefore, his creation, Psychoanalysis, drew nourishment. As happens with any look into the past, many myths become evident as such and, at the same time, are dissolved by looking at the historical facts that involve them. For example, one can see how the idea that hysterical patients were despised by physicians as simulators is false. Or, it can be seen that Freud was never the first to listen to these supposed patients ignored by physicians or that he was not the first or the only one to consider sexuality to explain hysteria. These and many other myths, such as that patients were treated by provoking them to orgasm, are easily debunked in this book. This book is obviously addressed to anyone interested in knowing, with accuracy and detail, what hysteria consisted of, as well as to those interested in seeing the reality behind the mythical foundations of Psychoanalysis, since it was born out of hysteria and to which it dedicated its existence. Patients with hysterical conversion present with striking physical symptoms such as weakness, sensory disorders or memory loss, that suggest a neurological disease but which show no evidence of brain and central nervous system damage. Although it is now over one hundred years since Breuer and Freud published their seminal Studies on Hysteria(1895) the story of hysteria remains controversial - even its existence as a viable clinical entity has been repeatedly questioned. Despite renewed interest over the past decade, most publications report little or no empirical research from the cognitive or clinical neurosciences. This is surprising given that the explanation of hysteria is still one where "the very notions of mind and body, and the boundaries and bridges between them are constantly challenged and reconstituted" (Porter, 1993). The rush to explain hysteria in terms of psychodynamics has so far proved elusive. Rather than developing further theories of hysteria, it is essential to charcterise those domains of normal volition and motor and sensory control that may be impaired, and from which it is possible to interpret observed symptoms. Only then will it be possible to provide a cognitively motivated account of how psychological mechanisms can translate (convert) into physical symptoms. As in other areas of psychiatry, it seems beneficial when explaining psychiatric phenomena to consider whether impairment to normal psychological phenomena can be used to construct a rational account of the underlying pathology.
