El proyecto Garantia de Calidad presenta un estudio sobre la prestacion de servicios de atencion integral a la Mujer que se brinda en dos municipios del departamento de Jinotega: El Cua y San Jose de Bocay y dos municipios del departamento de Matagalpa: Rio Blanco y Waslala. El estudio abarca un diagnostico sobre la estructura y funcionamiento de estos servicios, la valoracion de la competencia tecnica del personal de salud y el manejo de las emergencias obstetricas. La metodologia utilizada incluyo la aplicacion de un test de conocimientos dirigidos al personal de salud el cual se baso en el contenido tecnico de la Normas de Atencion Prenatal, Parto de Bajo Riesgo y Puerperio elaborado por el Ministerio de Salud. La muestra para el test de conocimiento fue definida de manera no probalistica e incluyo a 60 profesionales y tecnicos de la salud 57.7 porciento del universo. Se revisaron un total de 306 HCPB provenientes tanto de los puestos como de los centros de salud estudiados. Los resultados indican que los municipios beneficiados presentan como fortaleza la existencia de una organizacion de servicios que incluye la participacion de la direccion municipal hacia la parte comunitaria no se atiende con prioridad. En cuanto a las condiciones ambientales de las clinicas y sala de espera, en la mayoria de los casos se afecta el derecho de la paciente a la privacidad visual y auditiva. Los recursos humanos presentan la desventaja de tener poca capacitacion sobre la atencion integral de la mujer de manera ambulatoria. En su mayoria el personal es joven y, en el caso particular de los medicos se suma el pertenecer a los grupos que realizan servicio social por ano. En cuanto a la aplicacion de las normas se recomienda establecer acciones de obligatorio cumplimiento basado en los recursos locales, asi mismo se recomienda se preparen materiales auxiliares para el personal de salud que sirven como recordatorio para el fiel cumplimento de las normas ... Malaria remains an important cause of illness and death in children and adults in countries in which it is endemic. Malaria control requires an integrated approach including prevention (primarily vector control) and prompt treatment with effective antimalarial agents. Malaria case management consisting of prompt diagnosis and effective treatment remains a vital component of malaria control and elimination strategies. Since the publication of the first edition of the Guidelines for the treatment of malaria in 2006 and the second edition in 2010 all countries in which P. falciparum malaria is endemic have progressively updated their treatment policy from use of ineffective monotherapy to the currently recommended artemisinin-based combination therapies (ACT). This has contributed substantially to current reductions in global morbidity and mortality from malaria. Unfortunately resistance to artemisinins has arisen recently in P. falciparum in South-East Asia which threatens these gains. This third edition of the WHO Guidelines for the treatment of malaria contains updated recommendations based on a firmer evidence base for most antimalarial drugs and in addition include recommendation on the use of drugs to prevent malaria in groups at high risk. The Guidelines provide a framework for designing specific detailed national treatment protocols taking into account local patterns of resistance to antimalarial drugs and health service capacity. It provides recommendations on treatment of uncomplicated and severe malaria in all age groups all endemic areas in special populations and several complex situations.
