At its 279th Session (November 2000) the Governing Body of the International Labour Office decided to place an item on the recording and notification of occupational accidents and diseases, including the possible revision of the list of occupational diseases, Schedule I to the Employment Injury Benefits Convention, 1964 (No. 121), on the agenda of the 90th Session (2002) of the International Labour Conference, with a view to standard setting under the single-discussion procedure. The Governing Body also indicated that the development of a mechanism for regularly updating the list of occupational diseases should be examined by the Conference as part of the above agenda item. Owing to the limited time frame for the preparation of this law and practice report, it has been prepared on the basis of materials provided by member States in reply to past surveys, meetings of experts and other sources available at the International Labour Office.

This book presents an innovative method to improve workers’ health and prevent occupational accidents: the Change Laboratory, a method of formative intervention that enables the organization's participants to identify, with the help of facilitators, the historical and systemic origins of work processes anomalies (environmental problems, work safety and health, quality and productivity problems, problems related to labor relations, etc). This guide is designed to equip labour inspectors with the necessary skills to conduct effective investigations into occupational accidents, occupational diseases and other undesired events (such as dangerous occurrences and near miss/incidents) that could have led to personal injuries affecting workers or members of the public. It should be borne in mind that occupational accidents, diseases and other undesired events are preventable. Conducting an effective investigation will identify not only causal factors, but actions that would have prevented the event from occurring. While the ILO understands that other methodologies for conducting investigations are available, it believes that following the presented methodology will help inspectors to identify all the immediate and root causes of the event under investigation. This, in turn, will enable inspectors to help employers, enterprises and worker representatives to identify appropriate prevention/risk control measures to prevent a recurrence of the actions that led to the event under investigation, thereby improving occupational safety and health management.
