Background: Stroke survivors often experience various problems which may impact their qualities of life if not sufficiently addressed. Post-stroke problems beyond functional disabilities are often neglected and may not be picked up by caregivers/clinicians for further management.Aim: We aimed to study post-stroke complications reported by a cohort of ischemic stroke patients in Singapore and compare them with complication burden perceived by their caregivers using the Post-Stroke Checklist (PSC). Methods: We surveyed 40 ischemic stroke patients (median age 68.5 years) and their caregivers at 3 months post-stroke, to assess for post-stroke complications. The 13-questions PSC was administered separately to patients and their caregivers via telephone interview by trained study coordinators. Results: The mean problem number from patientsu2019 survey was 3.7 (u00b12.9), while it was 3.0 (u00b12.9) from the caregiversu2019 survey. There was good agreement (92.5%) for the questions on problems with activities of daily living. However, disagreements were greater for mood (27.5%), absence of secondary prevention (25%), cognition (25%), personal relationships (22.5%) and other challenges interfering with recovery (30%). Differing opinions in other areas were: mobility (17.5%), spasticity (20%), pain (17.5%), incontinence (20%), communication (20%), life after stroke (15%) and fatigue (17.5%). Conclusion: There was good concordance for some aspects of the PSC between patients and caregivers, which were more observable. However, there are other aspects with poor agreement possibly because they were more subjective and not demonstrated outwardly. It is important to consider these findings when screening consequences of stroke and to appropriately consider the opinions from caregivers.

Background The post-stroke checklist (PSC) was developed to identify consequences after stroke and was found to be feasible for use in the Singapore population. Of note, there are 4 domains on the PSC which are subjectively reported by the patient, namely pain, life-after-stroke, mood and fatigue. AimWe aim to study the burden, predictors using baseline variables and associations with 3-month functional outcomes of the 4 subjective post-stroke complications on the PSC, in a prospective cohort of ischaemic stroke patients admitted to a single tertiary hospital in Singapore. MethodologyThis is a prospective study of ischaemic stroke patients admitted to Singapore General Hospital from December 2016 to June 2017. Baseline demographics, stroke phenotypes and socio-economic factors were collated. At 3 months post-stroke, a structured telephone call interview was conducted by blinded trained study coordinators. The post-stroke checklist was administered and modified Rankin scale (mRS) was assessed for the functional status. Results Among the 145 ischaemic stroke patients (median age 62, IQR54-71, 67.6% male), the prevalence of issues with pain, life after stroke, mood and fatigue were 21.4%, 32.4%, 31% and 31% respectively. Predictor of issues with pain, mood, fatigue and life-after-stroke was associated with mRS at 3 months (p

A practical textbook, based on a problem-oriented workflow, that will improve patients' likelihood of full recovery from stroke and prevent future strokes from occurring Stroke is the leading cause of adult disability and is in the top five causes of death globally. Warlow's Stroke: Practical Management, 4th Edition takes a problem-oriented approach and addresses the questions posed by a stroke patient in the order they are likely to present in clinical practice, for instance, 'Is it a stroke?', 'What sort of stroke?', 'What caused it?', and 'What can be done about it?'. Beginning with chapters phrased as questions, the book walks the reader through a standard clinical workflow, exploring the practical skills and assessment required at each stage of patient management. Early chapters cover: locating the vascular lesion, identifying the involved arterial territory, the role imaging should play, and the application thereof. Subsequent chapters look at what causes a transient or persistent ischemic event, an intracerebral hemorrhage and a subarachnoid hemorrhage. Unusual causes of ischemic stroke and transient ischemic attack are also covered. The book then presents a practical approach to the management of stroke and transient ischemic attack; offers specific treatments for acute ischemic stroke and aneurysmal subarachnoid hemorrhage; provides ways for professionals to prevent first or recurrent stroke; and more. Final chapters of the book discuss rehabilitation after stroke, how patients and carers can be supported in the short term and long term, prevention of recurrent stroke, and the organization of stroke services. Warlow's Stroke: Practical Management, 4th Edition Follows clinical workflow for stroke analysis Features evidence-based approach throughout Offers practical application aimed at improving patient outcomes Written and edited by internationally renowned experts in the field An essential resource for all practitioners involved in the care of patients who suffer from cerebrovascular disease, but particularly suitable for neurologists, residents, geriatricians, stroke physicians, radiologists and primary care physicians.
