A virtual workshop titled Integrating Systems and Sectors Toward Obesity Solutions, held April 6, 2020 (Part I), and June 30, 2020 (Part II), was convened by the Roundtable on Obesity Solutions, Health and Medicine Division, National Academies of Sciences, Engineering, and Medicine. The workshop introduced the concept of complex systems and the field of systems science, and explored systems science approaches to obesity solutions. Speakers provided an overview of systems science theories, approaches, and applications, highlighting examples from within and outside the obesity field. Presentations and discussions examined complex systems in society that have the potential to shape public health and well-being, and considered opportunities for systems change as they relate to obesity solutions. Specifically, the workshop explored factors that can influence obesity - such as (in)equity, relationships, connections, networks, capacity, power dynamics, social determinants, and political will - and how these factors can impact communications and cross-sector collaboration to address obesity. This publication summarizes the presentations and discussion of the workshop.

Today’s children may well become the first generation of Americans whose life expectancy will be shorter than that of their parents. The culprit, public health experts agree, is obesity and its associated health problems. Heretofore, the strategy to slow obesity’s galloping pace has been driven by what the philosopher Karl Popper calls ‘‘the bucket theory of the mind. ’’ When minds are seen as containers and public understanding is viewed as being a function of how many scientific facts are known, the focus is naturally on how many scientific facts public minds contain. But the strategy has not worked. Despite all the diet books, the wide availability of reduced-calorie and reduced-fat foods, and the broad publicity about the obesity problem, America’s waistline continues to expand. It will take more than food pyramid images or a new nutritional guideline to stem obesity’s escalation. Albert Einstein once observed that the significant problems we face cannot be solved at the same level of thinking we were at when we created them, and that we would have to shift to a new level, a deeper level of thinking,tosolvethem. For the first time in decades, promising news has emerged regarding efforts to curb the obesity crisis in the United States. Obesity rates have fallen among low-income children in 18 states, the prevalence of obesity has plateaued among girls, regardless of ethnicity, and targeted efforts in states such as Massachusetts have demonstrably reduced the prevalence of obesity among children. Although the reasons for this turnaround are as complex and multifaceted as the reasons for the dramatic rise in obesity rates in recent decades, interventions to improve nutrition and increase physical activity are almost certainly major contributors. Yet major problems remain. Diseases associated with obesity continue to incur substantial costs and cause widespread human suffering. Moreover, substantial disparities in obesity rates exist among population groups, and in some cases these disparities are widening. Some groups and regions are continuing to experience increases in obesity rates, and the prevalence of severe obesity is continuing to rise. The Current State of Obesity Solutions in the United States is the summary of a workshop convened in January 2014 by the Institute of Medicine Roundtable on Obesity Solutions to foster an ongoing dialogue on critical and emerging implementation, policy, and research issues to accelerate progress in obesity prevention and care. Representatives of public health, health care, government, the food industry, education, philanthropy, the nonprofit sector, and academia met to discuss interventions designed to prevent and treat obesity. The workshop focused on early care and education, schools, worksites, health care institutions, communities and states, the federal government, and business and industry. For each of these groups, this report provides an overview of current efforts to improve nutrition, increase physical activity, and reduce disparities among populations.
