Introduction: The role of patent foramen ovale (PFO) closure to prevent ischemic stroke recurrence remains controversial. Its efficacy for the prevention of recurrence after cryptogenic stroke has been proven in recently published clinical trials, especially in young patients when large shunts are observed. Children have been excluded from these trials.Methods: Case report.Results: A nine years old girl had a cryptogenic ischemic stroke in the left thalamus. Cardioembolic sources, carotid stenosis or thrombophilia were not found. She was discharged without sequelae and started treatment with aspirin. Four years later, she was admitted to hospital with sudden onset dizziness, vomiting and right limb and gait ataxia. Magnetic resonance showed an acute right cerebellar infarction. She had favourable clinical outcome in few days with complete remission of symptoms. Transcranial doppler and transesophageal echocardiography showed presence of PFO with large interatrial shunt (>30 microbubbles). PFO closure was performed during admission. She was discharged on dual antiplatelet therapy and she was switched to clopidogrel alone after two months. No further recurrence have been recorded during follow up. Conclusions: In selected cases, children with cryptogenic stroke may be considered to perform PFO closure to prevent ischemic stroke recurrence.

Background and Aims:The patent foramen oval(PFO) is present approximately 50% of cryptogenic strokes. Recent clinical trials supported the endovascular closure of the patent foramen oval as a secondary prevention treatment in groups of patients and demonstrated less stroke recurrence than medical treatment.Methods:We performed a retrospective analysis of patients with cryptogenic stroke and findings of PFO in the echocardiographic tests followed in our neurologic department from 2012 to 2017. We recorded epidemiological data, diagnostic findings, treatments, follow-up and recurrence, in both groups of pharmacological treatment and non-invasive closure.Results:A total of 259 patients with cryptogenic stroke were evaluated, 59 patients presented a diagnosis of paradoxical stroke caused by PFO after causal evaluation by TOAST and ASCOD scales. Average age was 49+/- 8 years. Median value of ROPE score was 7. In 73% cases had an associated septum aneurysm (ASA) or massive right-left shunt, only 11% were moderate-mild and isolated. The recurrence rate of stroke due to large vessel occlusion in patients with PFO was less 2%. About 91% presented a good functional recovery in the third month (Rankin 0-2)The closure of the PFO was performed in 34% without complications, no variable showed significant differences. In this interventional group there was no recurrence in the mean follow-up of 3 yearsConclusions:Strokes caused by PFO present a very good prognosis with a very low recurrence in the medium/long-term in small series such as ours.About 70% of these patients with patent ovale foramen+ASA or massive shunt could be candidates to PFO closure.
