Background: In the setting of neurocritical care, percutaneuos tracheostomy technique(PDT) indications and advantages are recognized; early tracheostomy decreases both pulmonary morbidity and critical care resource utilization.Conversely to early complications, the late outcome of PDT is less studied.The aim of the study was to verify prospectically the rate of late complications in a cohort of patients submitted to one year outcome evaluation after severe traumatic brain injury (TBI) and who underwent PDT in the acute phase.Materials and Methods: 247 TBI patients tracheostomized in the acute phase were evaluated after 1 year.All tracheostomies were performed in the ICU using the Ciaglia Percutaneous Tracheostomy Set with endoscopic guide.At one year follow up disability was quantified by means of the extended Glasgow Outcome Scale and PDT outcome was focused on symptom interview and the physician examination with the cutaneous scar assessment.After this initial screening the patients symptoms severity were was summarized according their relevance in pauci symptomatic (1 symptom minimally significant), clearly symptomatic (2 or more symptoms presents or only one major intensity or need of surgical or medical treatment). Results and Discussion: Tab 1 describes patients characteristics. Tab 2 shows the late PDTcomplication rate. The overall rate and the severity of complications seems to be associated with poorer neurological outcome and this correlation is significative (Fig 1) Time of cannulation is longer in patients with poorer recovery (Fig.2). Conclusion: Late symptomatic complications seem to be associated with unfavourable neurological outcome. The longer time of cannulation in patients with poor neurological outcome is known to be associated to delayed complications.

"Tracheostomy is one of the simplest, most valuable operations in surgical practice. In tracheostomy an opening is made in the anterior wall of the trachea and this opening is used for insufflation of oxygen- air mixture into the lungs. A tracheostomy may be performed on an emergency or elective basis; it may be a temporary or a permanent one. Over the past several years the indications for a tracheostomy have radically evolved. Earlier tracheostomies were mostly done to relieve an upper airway obstruction; now tracheostomies are mostly done in critical care settings to facilitate mechanical ventilation. Open surgical tracheostomy (OST) is usually done in an operation theatre. A detailed description is given in the relevant chapter of the book, but a certain salient point needs to be emphasized here. First and foremost, neck anatomy is to be clearly defined and structures should be identified, as most of the complications occur when the surgeon deviates from the midline. Percutaneous tracheostomy (PCT) has certain distinct advantages over open surgical tracheostomy (OST); PCT is faster to perform, easier to learn and can be performed at the point of care. In most adult ICU patients, a percutaneous tracheostomy (PCT) is the preferred technique unless contraindicated. Compared to surgical tracheostomy it leads to significantly lesser wound infections, but it is associated with a higher rate of accidental decannulation and obstruction. Use of real time ultrasonography while performing percutaneous tracheostomy has also helped in better identification of tracheal rings and hence in improving its safety for obese patients. Tracheostomies are usually straightforward, but complications still occur, often due to improper technique. Complications may be due to structural damage to vital structures of the neck leading to hemorrhage, tracheal damage, esophageal damage, failure to proceed, air embolism, aspiration, hypoxemia, tube misplacement, pneumothorax, tracheal stenosis, trachea-arterial and trachea-esophageal fistula. This book encompasses many clinically relevant scenarios in which a tracheostomy may be needed. However, certain situations demand special expertise; tracheostomy in pregnancy is one such situation. Title. u201cHandyTraku201d: an innovative percutaneous tracheostomy creating deviceObjective. To evaluate the efficacy of u201cHandyTraku201d in creating a percutaneous stoma opening for tracheo-cannula insertion.Introduction. Percutaneous tracheostomy has become a frequent procedure within the intensive care environment and various alternatives for tracheo-stoma creation exist on the market. However, these devices have been associated with both acute and long-term complications, such as: excessive bleeding, posterior wall tracheal perforation, cartilage ring fracture, tracheal stenosis and others.Our hypothesis is that the unique type of dilation system which characterizes u201cHandyTraku201d, would create a less traumatic and more regularly shaped stoma, without the necessity to exert excessive pressure on the trachea or to insert devices deeply within the trachea. These advantages may have the potential o reducing the complications previously mentioned. Materials and Methods. In order to evaluate the efficacy of u201cHandyTraku201d in creating a percutaneous tracheo-stoma for tracheo-cannula placement, the device was tested on 4 cadavers after receiving authorization. The device consists of four 15mm dilator blades which, once inserted through the initial opening performed by a small introducer, would be uniformly opened by a ring-shaped mechanism through the clamping of two handles. This creates a circular shaped stoma, large enough for the passage of available tracheo-cannulas.Results. Eight tests were performed on 4 cadavers with the creation of a total of 8 stomas, all of which were circular in form without any tissue tearing or damage. However, in one test, the stoma was not sufficiently deep enough due to excessive subcutaneous tissue of the neck, making the dilators insufficient in length and not long enough to penetrate the anterior tracheal wall. These experiments were useful and enabled us make to the necessary modifications, (length of dilator blades from 10mm to 15mm in length).Conclusions. HandyTrak has the potential of representing a useful device for percutaneous tracheo-stoma device creation. Further studies performed on cadavers, and ultimately, patients, need to be performed in order verify the safety, time consumption and efficacy of this device before itu2019s introduction into clinical practice.
