A need for a more scientific approach to medical glove design, which incorporated performance requirements such as dexterity and tactility, was identified from discussions with manufacturers and a review of relevant literature. Based on the results of a review of existing test methods and interviews with a wide range of practitioners, a number of existing tests were identified for development and a number of new tests were proposed. The test apparatus and methods were designed, refined and validated with small groups of participants, allowing recommendations to be made for a battery of realistic, repeatable tests by which medical glove performance can be comprehensively characterised. The recommended tests covered three main areas of performance: manual dexterity, tactility, and grip and friction. As well as existing tests, including the Purdue Pegboard Test, the Crawford Small-Parts Dexterity Test and the Semmes-Weinstein Monofilaments, new tests were developed that better simulated the tasks carried out by practitioners, including a suturing test, the Simulated Medical Examination Tactility Test, the Pulse Location Test and the Roughness Perception Test. Apparatus was also designed to measure the effect of gloves on grasping forces and to compare static frictional properties of gloves. Grasp force and friction measurements were taken for examination gloves using human subjects and with a specially-designed anthropomorphic device. The results were compared with those obtained using a number of other friction measurement methods. There was little consistency between the test results, and none gave a definitive answer as to which glove produced the highest friction in any given situation. Further development of the apparatus and validation of the method was recommended, as well as a more comprehensive study of glove friction and the effects of lubrication. As part of the validation of the selected methods, analysis was carried out into the effect of glove material, thickness and fit on performance and the relationship between perceived and measured performance. Initial results suggested that glove fit had a greater effect on dexterity than tactility, with looser gloves reducing dexterity and tighter gloves reducing tactility. Glove thickness was found to be a significant factor in tactility, and in manual dexterity, where tactile feedback is required; thicker gloves and 'double-gloving' produced a reduction in tactility compared to thinner, single-layered gloves, and hence affected the ability to manipulate objects. Analysis of user perception of performance and of the effect of glove material properties did not produce clear trends. However, initial findings suggested that, contrary to user perception, natural rubber latex did not perform significantly better than alternatives such as nitrile and vinyl. A number of possible explanations for the discrepancy were proposed, and recommendations were made for future work with a larger sample size, including analysis of stress and fatigue levels and performing tests in lubricated conditions.

The WHO Guidelines on Hand Hygiene in Health Care provide health-care workers (HCWs), hospital administrators and health authorities with a thorough review of evidence on hand hygiene in health care and specific recommendations to improve practices and reduce transmission of pathogenic microorganisms to patients and HCWs. Cold-induced manual performance decrements pose a serious problem for military cold weather operations. A series of experiments conducted at the Naval Submarine Medical Research Laboratory, Groton, CT has been based on the notion that the amount of decrement, and possibly the best means for reducing that decrement, depend on the specific motor abilities required to perform a given task. Based on this notion, experiments have been performed using a battery of 'pure-ability' tasks so that ultimately results could be generalized to any task. The study conducted here had the following goals: (1) to complement the previous studies in terms of the cold temperatures tested, and obtain profiles of performance of decrements and cold-injury dangers across a range of cold temperatures; (2) to assess the wearing of gloves and temperature-specific training as potential methods of reducing or eliminating cold-induced manual performance decrements; and (3) to validate and compare results previously and currently obtained using 'pure-ability' tasks with those obtained using operational tasks. A battery of five pure ability and four operational tasks were administered to six four-man groups of U.S. Marines. Three groups practiced the tasks at room temperature on the first of three training days and at -10 C on the other two training days, while the other three groups practiced at room temperature on all three days.

The purposes of this study were (1) to measure the ability of medical specialists to perform 9 selected basic medical tasks while in fatigue uniform, MOPP-4, or MOPP-4 with a tactile glove, (2) to measure learning effects on performance of these tasks, (3) to determine the degree of which the ability to perform the tasks is degraded by MOPP-4 or MOPP-4 with the tactile glove, and (4) to obtain preliminary durability and user satisfaction data on the tactile glove. Subjects performed the tasks in fatigues, MOPP-4 with tactile glove, and MOPP-4 for six consecutive duty days, thus each subject served as his/her own control. Subjects were able to perform all of the tasks under all conditions on all test days. Subjects improved their performance speed significantly, while making fewer errors. The bulk of the improvement in performance occurred in the first 3-4 days of the study in fatigues, MOPP with tactical gloves, and MOPP-4. At the end of the study, subjects functioned at 77% of fatigue level in MOPP with tactical gloves and at 64% of fatigue level in MOPP-4. It may be advisable to alter manning patterns in order to maintain work output. Tactical gloves significantly improved performance, but have relatively low durability, with 15 of 18 damaged by the end of the study. They are of considerable value for tasks which require the extra dexterity.

Confronted with worldwide evidence of substantial public health harm due to inadequate patient safety, the World Health Assembly (WHA) in 2002 adopted a resolution (WHA55.18) urging countries to strengthen the safety of health care and monitoring systems. The resolution also requested that WHO take a lead in setting global norms and standards and supporting country efforts in preparing patient safety policies and practices. In May 2004, the WHA approved the creation of an international alliance to improve patient safety globally; WHO Patient Safety was launched the following October. For the first time, heads of agencies, policy-makers and patient groups from around the world came together to advance attainment of the goal of "First, do no harm" and to reduce the adverse consequences of unsafe health care. The purpose of WHO Patient Safety is to facilitate patient safety policy and practice. It is concentrating its actions on focused safety campaigns called Global Patient Safety Challenges, coordinating Patients for Patient Safety, developing a standard taxonomy, designing tools for research policy and assessment, identifying solutions for patient safety, and developing reporting and learning initiatives aimed at producing 'best practice' guidelines. Together these efforts could save millions of lives by improving basic health care and halting the diversion of resources from other productive uses. The Global Patient Safety Challenge, brings together the expertise of specialists to improve the safety of care. The area chosen for the first Challenge in 2005-2006, was infection associated with health care. This campaign established simple, clear standards for hand hygiene, an educational campaign and WHO's first Guidelines on Hand Hygiene in Health Care. The problem area selected for the second Global Patient Safety Challenge, in 2007-2008, was the safety of surgical care. Preparation of these Guidelines for Safe Surgery followed the steps recommended by WHO. The groundwork for the project began in autumn 2006 and included an international consultation meeting held in January 2007 attended by experts from around the world. Following this meeting, expert working groups were created to systematically review the available scientific evidence, to write the guidelines document and to facilitate discussion among the working group members in order to formulate the recommendations. A steering group consisting of the Programme Lead, project team members and the chairs of the four working groups, signed off on the content and recommendations in the guidelines document. Nearly 100 international experts contributed to the document (see end). The guidelines were pilot tested in each of the six WHO regions--an essential part of the Challenge--to obtain local information on the resources required to comply with the recommendations and information on the feasibility, validity, reliability and cost-effectiveness of the interventions.
