Background: ufffcTo evaluate the incidence and risk factors in well-maintained subjects with dental implants with bioceramic molecular impregnated treatment (Osseanu00ae, Intra-Lock International, Boca Raton, FL, USA) in function in a 5-year period. Aim/Hypotesis: Peri-implantitis is a plaque-induced inflammatory reaction characterized by presence of probing pocket depth and bone loss. There are several implant surface topographies available in the market as well as some hypothesis that showed specific surface topographies can be more susceptible than other. Matherials and Methods: Two hundred and sixty-eight implants of 3.3-5.0 mm width and 8-13mm length were installed in 89 subjects between 2010 and 2011. Implants were functionally loaded after 60-day post-surgery with a provisional restoration and thereafter final restorations in ceramic were delivered. Radiographic bone loss was measured on periapical radiographs after 4, 12, and 60 months. Periodontal parameters as percentage of visible plaque, marginal bleeding, probing depth, clinical attachment level, bleeding on probing and suppuration were also evaluated at 60 months. Technical problems as ceramic fracture and loose of abutment screw were evaluated. The cumulative survival and implant-restoration success were assessed using the Kaplan-Meier survival estimator; Chi-square test was applied to evaluate correlations between the study variables. Results: The mean of radiographical bone loss after 60 months was 1.03mm (u00b1 0.30). Six dental implants were lost due to Peri-implantitis (PD >5mm; BoP and/or sup; Bone loss >4mm); 4 out 6 dental implants were lost in smokers with history of periodontitis. A total of 11 dental implants were lost after 5y - 95.89% of success rate. Incidence of peri-implantitis was 2.23% and mucositis 24.35% (PD>4mm, BoP and no bone loss). However, 4.30% of the remaining implants present risk for peri-implantitis (bone loss >3mm without BoP). Lower incidences of technical problems during the period of evaluation at 5 years were observed (11.23%). Conclusions and Clinical Implications: Within the limitation of this prospective study, dental implants with bioceramic molecular impregnated treatment placed in a well-maintained patient presented higher success rate and low incidence of peri-implant diseases after 5y follow-up.

Background: The clinical parameters together with multifactorial origin indicate a challenge for clinicians to correctly diagnose peri-implant pathology and define a patientu2019s risk for developing peri-implant diseases and management strategies against the occurrence of them. The identification of controlling risk factors combined with the assessment of multiple clinical parameters can be illustrated through prediction models in order to provide more accurate diagnostic information about peri-implant disease. Aim/Hypothesis: This study aimed to develop a predictive diagnostic model for peri-implant health and disease. For this purpose, predictor variables with the highest potential of impact were determined and implemented into three different data mining models. The models were compared to present the highest accuracy.Materials and Methods: This cross-sectional case-control study included a total of 216 patients previously treated with 542 dental implants. The patients and their implants were evaluated in the following six categories: patient-related health and behavioral factors, implant-site characteristics, surgery-, prosthesis- and esthetic-related factors and clinical and radiographic characteristics. The implants were classified into three groups (peri-implant health, peri-implant mucositis, and peri-implantitis) based on presence/absence of bleeding on probing (BOP), with or without suppuration (SUP), increase in probing depths and levels of marginal bone loss. Prediction models were developed using clinical parameters combined with possible risk indicators for peri-implant mucositis and peri-implantitis. In that sense, three different data mining methods [decision-tree (J48), logistic regression and multilayer perceptron) were compared to provide a better predictive model.Results: The prevalence of peri-implant mucositis and peri-implantitis was 40.2% and 35.3%, respectively. BOP, SUP, plaque index (PI), pink esthetic score (PES), maintenance therapy, medication use, active periodontal disease, the time between tooth extraction and implant placement, augmentation, type of prosthesis, implant function time, implant diameter, etiology of tooth loss, number of implants per patient and peri-implant soft tissue biotype were identified as potential predictor variables with the highest impact. J48 method, which is a decision-tree method, provided a better prediction of peri-implant health/disease than the logistic regression and MLP methods for the present dataset. In the predictive model of J48, the top level node was the u2018BOPu2019 variable. For the next split-levels, u2018maintenance therapyu2019 and u2018medication useu2019 were observed as strong predictive factors for peri-implant diseases.Conclusions and Clinical Implications: The present model created using the decision-tree method showed satisfactory predictive accuracy of peri-implant health, peri-mucositis and peri-implantitis. The model emphasized u2018BOPu2019 as a major predictive measure for peri-implant health and disease when evaluated with other possible risk indicators. The present outcomes highlight the importance of controlling patient-related factors such as maintenance therapy and medication use for preventing the development of peri-implant diseases.

Abstract: Aim Few data are available regarding the long-term impact of supportive implant therapy (SIT) on peri-implant diseases. We present long-term results on the possible effects of SIT on the peri-implant tissues. Material and methods In a private practice, 50 consecutive patients with 101 implants had attended an SIT programme for >6 years (SIT group). A second consecutive group comprised patients without SIT compliance (C group). We assessed pocket probing depth (PPD), bleeding on probing (BOP), bone loss (BL) and implant hygiene (QHI). Correlations between SIT compliance and the prevalence of peri-implant diseases were evaluated statistically. Results The inclusion criteria were met by 48 patients with 98 implants (SIT group) and 43 patients with 121 implants (C group). SIT group: Mucositis rate 30%; mean PPD 3.76 ± 0.86 mm; bone loss 1.02 ± 0.85 mm, peri-implantitis rate 4%. C group: Mucositis rate 68%; mean PPD 4.07 ± 1.18 mm; bone loss 1.53 ± 1.46 mm; and peri-implantitis rate 17%. Significant associations were found between SIT compliance and reduced peri-implant mucositis (p = .019), reduced highest PPD (p = .038) and peri-implantitis rates (p = .027). Conclusions Patients under SIT were associated with lower incidence of peri-implant diseases. Patients without regular maintenance might exhibit a 4.25-fold increased risk for peri-implantitis. Therefore, SIT programmes should be considered essential
