Background: Both hypo- and hypervolemia increase the risk for postoperative complications after major abdominal surgery. Fluid needs vary amongst patients depending on differences in preoperative dehydration, intraoperative physiology and surgical characteristics. Goal-directed fluid therapy (GDFT) aims to target the right amount of fluid administration in each patient by evaluating the effect of fluid boluses on haemodynamic parameters such as stroke volume. It has been shown to reduce postoperative morbidity and is generally recommended for high-risk surgery. The overall aim of this thesis was to evaluate whether more simple devices for GDFT result in clinical benefit, thus facilitating the application of GDFT in more patients. Aim: To compare performance and clinical benefit of pleth variability index (PVI), a noninvasive, easy-to-use device for GDFT, with the reference method of oesophageal Doppler; to evaluate methods for measuring preoperative dehydration and its effect on fluid handling by the body; and to confirm the expected clinical benefits of GDFT in patients undergoing oesophageal resection, a high risk procedure. Methods: In Studies I-III 150 patients scheduled for open abdominal surgery of at least 2 hrs were randomised to GDFT with either PVI or oesophageal Doppler. In the first half of the cohort, both monitors were connected to compare intraoperative performance. In 30 patients preoperative dehydration was analysed. In study IV 64 patients undergoing oesophageal resection were randomised to GDFT using pulse contour analysis or standard treatment. Results: The concordance between PVI and oesophageal Doppler for indicating the need for and effect of a fluid bolus was low, and both had only limited capacity to predict the effect of a fluid bolus. Both methods resulted in comparable amounts of fluid being administered and similar clinical outcome. Preoperative dehydration was limited but did impact on fluid handling. Patients receiving GDFT during oesophageal resection received more fluid and more dobutamine compared to controls, but this did not result in any clinical benefit. Conclusions: There are methodological issues as well as uncertainties about the clinical benefit of GDFT. We cannot recommend a strict application of any GDFT strategy, but suggest that its components should be incorporated in a more encompassing assessment of a patient’s fluid needs. The measurement, impact and treatment of preoperative dehydration need to be further clarified.

Plasma osmolality is an important determinant for fluid shift across the capillary wall. A decrease in plasma osmolality leads to tissue edema, which impairs tissue perfusion. Perioperative fluid infusion may affect the blood osmolality, because theoretical fluid osmolality is different that real [1]. Several studies have documented that crystalloids infusion affects plasma osmolality whereas colloid fluids are iso-oncotic [2]. The purpose of this study was to analyse the plasma osmolality in patients treated with crystalloids or gelatine infusions. Patients and methods. Adult patients undergoing elective knee arthroplasty surgery under spinal anaesthesia were enrolled. Patients with complicated perioperative period and/or those, who required massive fluid resuscitation following perioperative bleeding were excluded. Plasma osmolality was measured just before surgery (A - baseline), 3 hours after surgery (B) and 24 hours after surgery (C). Patients were divided into four equal groups (n = 20): R u2013 patients receiving Ringer solution, P u2013 Plasmalyteu00ae, S u2013 Sterofundinu00ae ISO and G u2013 Gelaspanu00ae. All fluids were administered at the dose 10 u2013 15 mL/kg body weight/h.Results. 80 patients (33 female and 47 male) aged 34 u2013 84 (mean 63 u00b1 12) were studied. Fluid administration is an important aspect of perioperative care in colorectal surgery. It has been largely guided by experimental evidence from the 1950s and extrapolated observations from injured soldiers during the Korean and Vietnam War. Recent improvements in perioperative care have led to renewed scrutiny of perioperative fluid management and challenged conventional wisdom with regards to the ideal quantity of fluid to be administered for patients undergoing colorectal surgery. Chapter one introduces this topic with discussion of optimised perioperative care, fluid and electrolyte physiology, composition of intravenous fluids and their clinical implications. It discusses the history of fluid therapy and concludes that avoidance of fluid overload -fluid restriction- and individualised fluid therapy are considered as ideal fluid regimens in colorectal surgery, thus outlining the direction of the rest of the thesis- comparison of the two and exploring their place within the perioperative environment. The prevalent heterogeneity in perioperative care and its influence on fluid administration is demonstrated in chapter two. Chapter three explores the feasibility of intraoperative fluid restriction and establishes a baseline for future comparative studies whilst also showing an important association between increasing fluid amounts and adverse clinical outcomes. Chapter four shows interest and equipoise concerning the use of individualised fluid therapy in clinical practice. It also demonstrates that the Oesophageal Doppler Monitor (ODM) is the most favoured instrument to conduct individualised fluid therapy. Chapter five is a systematic review of the methodology of the published trials exploring ODM-guided fluid therapy in colorectal surgery. It outlines the methodology and limitations of prevalent evidence and allows for the design of prospective studies. Chapter six is a prospective study of individualised fluid therapy in rectal surgery.
