The human personality is inextricably bound up with, among other things, the function of the central nervous system. Diseases and malfunctions of the brain, head injuries and neurosurgical operations can all result in permanently altered behaviour patterns. This interrelation between brain and behaviour is most clearly demonstrated in cases involving functional neurosurgery and severe traumatic lesions. Despite the fact that this interrelation represents an everyday challenge to the neurosurgeon, it is a question which receives less attention than it deserves in neurosurgical meetings. Given the scope and complexity of this topic, it is not possible to cover every aspect of it here: hence, discussion is limited to the impact on personality of injuries, language, epilepsy and psychosurgery. However, before considering the medical aspects, it was deemed necessary to try and arrive at a definition of "personality". This question was discussed by a number of philosophers representing various perspectives. Their diversity of viewpoints and conceptions greatly enriched the discussions.

This study is a pioneering effort done following a new direction in the UST Graduate School researches particularly in the field of neuropsychology. As pure or basic research, it aimed to determine the personality traits of Filipino patients with frontal lobe damage before and after brain injury. The researcher employed multiple case studies as a research strategy which used qualitative data to establish the pre-and-post injury personality traits of the participants with frontal lobe damage. Qualitative data are derived from CT scan/MRI findings, life history interview with the participants and significant others focusing on the pre-morbid personality functioning of the participants and the administration of neuropsychological tests namely Frontal Systems Behavior Scale (FrSBe) and Neuropsychological Behavior and Affect Profile (NBAP). These reflect the association between the pre-injury and post-injury personality traits and frontal lobe damage and to form opinions regarding the personalities of the participants. The 30 outpatients of neurosurgery service of a government hospital in Metro Manila and the 60 significant others of their choice who are usually their family members or co-workers were the participants of this study. Findings showed that majority of the frontal lobe damaged participants possesed these pre-injury personality traits low apathy, low disinhibition and low executive functionms, low indifference and low mania but high inappropriateness, high pragnosia and high depression. But after frontal injury and neurosurgery, they manifested low apathy, low inhibition, low executive functions, low inappropriateness, low indifference, low pragnosis, low depression and low mania.

Recent developments in the field of neurosurgery, specifically those dealing with the modification of mood and affect as part of psychiatric disease, have led some researchers to discuss the ethical implications of surgery to alter personality and personal identity. As knowledge and technology advance, discussions of surgery to alter undesirable traits, or possibly the enhancement of normal traits, will play an increasingly larger role in the ethical literature. So far, identity and enhancement have yet to be explored in a neurosurgical context, despite the fact that 1) neurological disease and treatment both potentially alter identity, and 2) that neurosurgeons will likely be the purveyors of future enhancement implantable technology. Here, we use interviews with neurosurgical patients to shed light on the ethical issues and challenges that surround identity and enhancement in neurosurgery. The results provide insight into how patients approach their identity prior to potentially identity-altering procedures and what future ethical challenges lay ahead for clinicians and researchers in the field of neurotherapeutics.
