Phúc trình mang số hiệu A/5630 là báo cáo của Phái đoàn Điều tra Liên Hiệp Quốc tại Nam Việt Nam (Report of the United Nation Fact-Finding Mission to South Viet-Nam) được soạn thảo bằng tiếng Anh, tiếng Pháp và tiếng Tây Ban Nha, là kết quả của một cuộc điều tra khách quan do Liên Hiệp Quốc tiến hành thông qua việc chỉ định các đại diện từ 7 quốc gia thành viên cùng một số nhân viên chuyên môn để hỗ trợ hoạt động điều tra. Phái đoàn điều tra này đã đến Nam Việt Nam ngày 24-10-1963 và đến sáng ngày 1-11 thì dự kiến sẽ hoàn tất công việc vào cuối ngày 3-11. Tuy nhiên, cuộc chính biến diễn ra trong ngày 1-11 đã làm thay đổi phần cuối kế hoạch, cũng như có thể là nguyên nhân khiến cho Phái đoàn không nhận được những tài liệu quan trọng mà Chính phủ ông Diệm đã hứa sẽ cung cấp. Ngoài ra, để chuẩn bị các phương thức và chương trình hành động sao cho khách quan và hiệu quả, trước đó phái đoàn cũng đã có 4 phiên họp trong thời gian từ ngày 14-10 đến 21-10-1963 tại New York.Bản Phúc trình A/5630, chỉ riêng phần Anh ngữ dài 93 trang khổ lớn, gồm 4 Chương với 191 phân đoạn (paragraphs) và 16 Phụ lục (Annexes), được phái đoàn trình lên Kỳ họp thường niên lần thứ 18 của Đại Hội Đồng Liên Hiệp Quốc, là tài liệu quan trọng để Đại Hội Đồng thảo luận và xem xét trong phạm vi Đề mục 77 (Item 77) theo Nghị trình Kỳ họp (Agenda) đã được Đại Hội Đồng thông qua trước đó, với tiêu đề chính là "Vi phạm nhân quyền ở Việt Nam" (The violation of human rights in South Viet-Nam).Trong thực tế, Đại Hội Đồng đã không tiến hành việc thảo luận Đề mục 77 như trong Nghị trình đã định.
